Description
The VA Connecticut Heath Care System 950 Campbell Avenue West Haven, CT 06516 Seek the following Services DESCRIPTION/SPECIFICATIONS/WORK STATEMENT AND SPECIAL SERVICES: a.Provision of Clinical Services. Contractor will provide on-call services for Therapeutic Apheresis (TA) and Cellular Therapy (CT) Services as ordered by the physician staff at VA Connecticut Healthcare System. Services will be provided seven days a week, 24 hours a day. b.Contractors are required to be properly licensed according to state laws that govern the location of their individual practices and the accrediting bodies that oversee their individual disciplines. They are also required to maintain compliance with applicable law, regulatory, and inspecting agencies. Providers will be credentialed to provide patient care services at VA Connecticut health System. Providers will have initial credentialing, 1 year recredentialing and every 2 years, thereafter. Providers must undergo appropriate training and supply competencies to reflect such training. Providers must have a current ACLS/BLS (must have a minimum of Adult Heart saver with AED) certification. c.Documentation will be made by Contractor directly into the VA's Computerized Patient Record System (CPRS) and will be done so in a timely manner consistent with VHA and Joint Commission regulations and requirements. Notes must be entered and signed on date services are provided. Contractor must attend CPRS training and show competency to navigate the electronic system. d.Clinical Services Pre/Post Opt Requirements: i.Contractor will receive and give SBAR (Situation, Background, Assessment and Recommendation) before and after service from/to Primary nurse. ii. Contractor will verify patient consent for treatment. iii.Contractor will reinforce education of procedure to patient iv.Contractor will verify patient's identity using two identifiers. v.Contractor will verify physician's orders for the treatment. vi.Contractor will take and monitor vital signs per hospital protocol and document in the vital signs package in CPRS vii.Contractor will complete and electronically sign a templated progress note on day of services rendered. e.Equipment and Supplies will be provided by contracted company. The cell separator will comply with all applicable regulatory standards. The company will maintain this equipment in accordance with the manufacturer's recommendations. All equipment will be available to VACT pre every use to obtain authorization for use within the Hospital per national JCAH standards. Clinical Engineering is available 24/7 for routine and urgent patient use. A copy of preventative maintenance documents will be supplied to VACT. The contractor will provide all disposable supplies required for services and maintain compliance with the hospitals waste disposal regulations and policies. f.Emergencies or Complications i.Contractor will immediately notify hospital staff of emergencies and complications ii.All medications will be ordered and supplied by hospital staff.
