Description: Description(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation F11PS01361, Deliver, Tail-Gate or Belly-dump spread, Class 5 Crushed Gravel on the Scenic Trail, at J. Clark Salyer National Wildlife Refuge, North Dakota is issued as a Request for Quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53, effective 04 August 2011. (iv) This procurement is being issued as a 100% SMALL BUSINESS SET-ASIDE IAW FAR 19.502-2(a). The North American Industry Classification System (NAICS) Code is 212322, Construction sand and Gravel beneficiating, with a size standard of 500 employees. (v) The Contractor shall provide the following items: LINE ITEM 0001 - The US Fish and Wildlife Service, J. Clark Salyer National Wildlife Refuge is requesting quotes for Class 5 Crushed Gravel to be delivered, tail-gate or belly-dump spread on the scenic trail at J. Clark Salyer National Wildlife Refuge on the portions of the road that are dry enough to spread. The remainder of gravel shall be stockpiled at the Refuge headquarters. Delivery/weight tickets shall be submitted with invoices. Quantity of Gravel: 1.Deliver and tail-gate or belly-dump spread 1,500 Tons of Class 5 Crushed Gravel on the Scenic Road at J Clark Salyer National Wildlife Refuge, North Dakota The price will be F.O.B. Destination Delivery: 15 September 2011 Delivery Location: U.S Fish and Wildlife ServiceJ Clark Salyer National Wildlife Refuge681 Salyer RoadUpham, North Dakota, 58789 (vi) The provision at 52.212-1, Instructions to Offerors - Commercial Items and 52.212-2, Evaluation - Commercial Items apply. Evaluation will be based on price. The price will be F.O.B.
