- solicitation pkg form pg 1 attachments solicitation pkg starts pg 2 solicitation letter The Albuquerque Area has a requirement for reference laboratory services in support of clinical and anatomical laboratory services and consulting clinicial pathologist to the clinical staff for all the health facilities located within the Albuquerque Area Indian Health Service Area, as well as some tribally operated health facilities. The Offeror must be a laboratory licensed under the Clinical Laboratory Improvement Amendments of 1988, (CLIA '88), Public Law 100-578. The Offeror's proposal must include the availability of a Consulting Pathologist, necessary personnel, supplies, equipment, courier services, and facilities to provide comprehensive services which will include but are not limited to cytology, surgical, pathology, microbiology, chemistry, hematology, immunohematology, serology and consulting pathologist in-person or available by phone or teleconference at each facility during any regulatory accreditation survey. The Government anticipates award of a single fixed-price, requirements type contract for a base year with four, one-year options to renewal. All proposals received under the referenced solicitation number will be evaluated and considered for award. The Government intends to evaluate and award a contract without discussions with Offerors (except for clarifications) pursuant to Federal Acquisition Regulation (FAR) Part 15.206(a), therefore the Offerors' proposal should submit its best proposal. Technical and cost/price proposals are of equal value, in the case of a technical tie, fair and reasonable cost/price will be the determing factors(s) for award. Refer to Solicitation Number 11-242-SOL-00031 when requesting a solicitation package. The Offerors are required to submit, by US Postal Service or hand delivered, hardcopies of their proposals to the following address: Albuquerque Area Indian Health Service, Division of Contracts and Grants Management, 5300 Homestead Road NE, Albuquerque, NM 87110. The RFP due date is close of business 4:30 MST by September 9, 2011. Late proposals will not be accepted unless they were mailed via USPS and post marked prior to the time and date of the due date.
