B.1 - SCHEDULE OF SERVICES - For: "AMBULANT TRANSPORTATION SERVICES" FOR THE PHOENIX VA HEALTHCARE SYSTEM 1. REQUIREMENTS: Contractor shall provide ambulatory, wheelchair and stretcher transportations services to VA beneficiaries of the Phoenix VA Health Care System and Clinics in accordance with all terms, conditions, provisions, schedules and the attached Scope of Work (SOW) and resultant contract herein. Contractor shall provide safe and comfortable ambulatory, wheelchair and stretcher transportation services on an as needed basis 24 hours a day, Seven Days a week, to include holidays, from and to the front doors of apartments, houses, and/or other areas designated by the Beneficiary Travel Office. The primary intent of this contract is for pre-arranged, non-emergency, lie-down, wheelchair and ambulatory transportation services. NOTE: This contract is NOT for emergency medical services. Vehicle driver will assist the patient, as necessary, to enter and exit vehicles, and enter, locate, and sign-in at appropriate clinic areas upon arrival at destination. Vehicles transporting VA beneficiaries will be dedicated exclusively to VA transports while performing services for the VA. The contractor may not carry non-VA contract beneficiaries or private pay patients with VA Beneficiaries. Contractor performance will be in conformance with the contract and all applicable Federal, State or local laws. 2. HOURS OF OPERATIONS: The majority of trips under this contract will be Monday through Friday, 5:00 am to 8:00 pm, exclusive of Federal Holidays. "After Hour" (8:01 PM - 4:59 AM) charges for transportation services required after the normal working hours (5:00 AM - 8:00 PM) shall apply IAW the stated Schedule pricing. 3. SPECIFIC INSTRUCTIONS FOR THIS "SOURCES SOUGHT" ANNOUNCEMENT The Government is attempting to determine the socio-economic status of companies capable of performing the above services. Accordingly, if interested, you must contact the below listed party and provide evidence of your status, location(s) where you currently perform these services, if you have at least three years performing similar services, and a statement of availability to perform these services in the Phoenix metropolitan area. Again, the Government is not just looking to determine your interest. We must know what your business status is. Without this we cannot determine whether or not a set-aside is appropriate for this requirement. 4. POINT OF CONTACT Please forward your responses to: Mr. Greg Manning at Gregory.Manning2@va.gov.
