- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation W67K2S11950003 is issued as a Request for Proposal. North American Industry Classifications System Code (NAICS) is423450, size standard is $6.5M. SPECIFICATIONS: 1.Four (4) each SimMan 3G (Leardal Mfg P/N 212-01001) complete with 12" monitor. Must include: SimMan 3G mannequin, software and license, operator tablet PC, patient monitor and software, patient cables, webcam, drug recognition kit, soft sided transportation cases and patient cases for Auto-mode. One year warranty. 2.Four (4) each SimMan 3G ValuPlus Platinum (Leardal Mfg P/N 212-VPLUSP5). To include: installation, extended five year warranty, on-site preventative maintenance, and loaner. 3.Two (2) each Intro to SimMan 3G On-Site training sessions (Leardal Mfg P/N 212-90150). To include: two full day on-site training sessions to eight participates. Travel included. 4.Two (2) each SimMan 3G Bleeding Modules (Leardal Mfg P/N 212-07450). To include: amputated arm, gunshot arm, amputated leg, gunshot leg, hardware kit, and DFU. 5.Two (2) each Simulated Nuclear, Biological, Chemical (NBC) modules (Leardal Mfg P/N 276-15001). 6.Two (2) each Bleeding trauma modules (Leardal Mfg P/N 381550). Note: Specified manufacturer only. Equivalents will not be accepted. Items shall be delivered and training provided at Utah Army National Guard Headquarters; 12953 South Minuteman Drive, Draper, Utah 84020. EVALUATION FACTORS: The contract type for this procurement will be a firm-fixed price award and will be made on the basis of best value to the Government, price and other factors considered. Selection will be based on the following evaluation factors, in order of importance: (1) Lowest price technically acceptable and (2) ability to meet delivery requirement of 3 October 2011. ADDITIONAL INFORMATION: In accordance with FAR 52.212-1(k), prospective awardees shall be registered and active in the CCR database prior to award. Offerors may obtain information on registration at www.ccr.gov. And also in accordance with FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, Offerors shall have completed registration on the ORCA website at https://orca.bpn.gov. In addition to the completion in ORCA, if the offeror is proposing items manufactured from other than the United States, they shall state the "Country of origin" with their offer.
