- DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Activity: Department of Veterans Affairs, VISN 4, Wilkes-Barre, PA Medical Center 1111 East End Boulevard, Wilkes-Barre, PA 18711. Justification for Other Than Full and Open Competition in reference to VA purchase request number: 693-11-4-179-0264. 2.Nature and/or Description of the Action Being Processed: Justification and approval for other than full and open competition for a new firm fixed price contract to be awarded for an Eyesi Cataract System (Eye Surgery Simulator). 3.Description of Supplies/Services Required to Meet the Agency ™s Needs: Purchase of one (1) Eyesi Surgical Simulator Platform to include the following Eyesi Cataract components; one (1) Eyesi CAT Surgery Interf. Pack including: one (1) Eyesi CAT hand-activ. Forceps. One (1) Eyesi CAT basic skills pack 1 including: Eyesi CAT anti tremor training, Eyesit CAT forceps training and Eyesi CAT Phaco basic skills training. One (1) Eyesi CAT basic skills pack 2 including: Eyesi CAT navigation training, Eyesi CAT bimanual training and Eyesi CAT chopping and cracking training. One (1) Eyesi CAT Capsulorhexis. One (1) Eyesi CAT Hydro Maneuvers. One (1) Eyesi CAT Phaco Divide and Conquer. One (1) Eyesi CAT Irrigation Aspiration. One (1) Eyesi CAT training courseware. One (1) Eyesi surgical warranty extension 2 years. Complete Eyesi Cataract System total price including above components for an estimated $142,900.00. Purchase of one (1) ES Gold software system for 36 months to include all Eyesi surgical software updates to existing modules and operating system for 36 months and all new Eyesi surgical simulation modules released for the purchased hardware during the 36 months term for an estimated $29,670.00. Purchase of one (1) ES transport containers for an estimated $3,400.00. Grand estimated total of acquisition for $175,970.00 Estimated delivery date on or before September 30, 2011. 4.Statutory Authority Permitting Other than Full and Open Competition: FAR 6.302-1 -- Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. 41 USC §253(g), as implemented by FAR 6.302. 5.Demonstration that the Contractor ™s Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): VRmagic developed the ophthalmic surgery simulator EYESi. Ophthalmic surgical procedures can be performed in a life like virtual environment. EYESi allows training of cataract as well as vitreo-retinal surgery at an exceptional degree of realism. EYESi provides high quality medical education. The simulator will allow the Ophthalmology staff to improve the surgical care that is being provided to our veterans. It will allow staff to decrease surgical time by allowing practice time outside of actual live surgery. It will allow for a simulation of surgical complications outside of the operating room to better prepare for them should they occur. It will allow for decreased complication rates as existing publications suggest up to an 80% reduction in complication rates. The Eyesi surgical simulator platform has been purchased at several VAMC ™s throughout the country and has proven to be a successful training addition to residency education. Since one of the stated goals of the VA is education, the addition of this type of equipment will keep the Ophthalmology clinic staff in the forefront of the educational direction. As a comparison in 2007, 25-30% of all residency programs were using surgical simulation within their curricula. The efficiencies will allow for a decrease in operating room time per case with the ability to do more cases in a given time period. Data indicates that there is about a 40% reduction in surgical time. Additional cost analysis can be furnished; however, it will be dependent upon operating room turnover time, number of cases per year, anticipated complication rates, waiting times for surgery, etc. 6.Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: VRmagic ™s Eyesi Surgical Simulator Platform is a highly complex and specialized piece of equipment that was developed by VRmagic. A search in the VetBiz registry and the CCR Dynamic Small Business search yielded no potential sources who can provide or manufacturer this technical simulator. In addition, there is no GSA schedule holders listed who could provide the simulator. 7.Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: The educational, safety and efficiency values of this procurement cannot be underestimated. The potential safety outcome by training in a virtual setting will be worth the financial cost in of itself. As stated above, industry research suggests up to an 80% reduction in complication rates by utilizing the Eyesi simulator, as well as a 40% reduction in surgical time. In addition, simulators play an essential role in evidence-based training. By guaranteeing reproducibility in training, simulators provide exactly identical tasks for trainees and deliver objective skills assessment through measurable performance data. Realistic simulation increases experience at low risk and cost. Hundreds of surgical procedures can be performed each day without ever putting a patient at risk. At the same time, simulation provides objective assessment and gives the medical educator high confidence in the trainee's abilities before entering the operating theatre. 8.Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: A search in the VetBiz registry and the CCR Dynamic Small Business search yielded no potential sources who can provide or manufacturer this technical simulator. In addition, there is no GSA schedule holders listed who could provide the simulator.
