Description

THIS NOTICE IS PROVIDED FOR INFORMATION PURPOSES ONLY. THIS OPPORTUNITY IS AVAILABLE ONLY TO CONTRACTORS ELIGIBLE UNDER SERVICE DISABLED VETERAN OWNED SMALL BUSINESS SET ASIDE (SDVOSB) Set Aside. The Veterans Integrated Service Network #04 (VISN 04) Contracting Office / Altoona (James E. Van Zandt VAMC) intends to issue an Invitation for Bid (IFB) to award a single, firm fixed-price contract for Construction Services related to the Construction of Project, ARK (Anesthesia Record Keeping System) / CIS (Clinical Information System) Wall Mount # 503-11-204 (i.e., installation of twenty-one (21) mounting plates and equipment brackets installed on the walls in various locations throughout building no. 1 (i.e., VA Medical Center). The brackets are to carry a computer monitor that is mounted on to an arm that swings from the bracket). All construction must be in compliance with applicable codes, VA policy/standards, and the terms of this contract. The contractor shall provide all tools, travel, trades, labor, materials, permits, licenses and supervision for the construction. The project is located at the James E. Van Zandt VA Medical Center, 2907 Pleasant Valley Boulevard Altoona PA 16602-4337. Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work as required by the drawings and specifications. The period of performance is 90 calendar days from receipt of the Notice to Proceed. The Solicitation, Specifications and Drawings will be available at http://www.fedbizopps.gov. Amendments to the solicitation will also be posted on this website. Furthermore, all respondents must be registered in CCR data base, see http://www.ccr.gov and complete online Representations and Certifications at http://orca.bpn.gov along with Vets 100 at http://vets.dol.gov/vets100/. Bidders must be registered in vetbiz at http://www.vetbiz.gov/. Your firm must be visible in the VetBiz.gov registry to be considered eligible to compete under the SDVOSB Set Aside Program. All parties must obtain amendments to the solicitation through the web site. Hard copies will not be mailed. Interested parties must go to the http://www.fedbizopps.gov web site to be entered on the Interested Vendors Mailing List and to obtain copies of the Interested Vendors Mailing List. The contract will be awarded in accordance with the procedures specified in the solicitation. The solicitation will be issued as an Invitation for Bid (IFB) conducted in accordance with FAR Part 14. This project is being solicited as a TOTAL 100 % SERVICE DISABLED VETERAN OWNED SMALL BUSINESS SET-ASIDE (SDVOSB). The North American Industry Classification System (NAICS) Code for this procurement is 236220, with a small business size standard of $33.5 million in annual receipts for the past three years. The Magnitude of Construction cost for the above referenced project is between $25,000.00. to $100,000.00. The solicitation package, amendments, and site visit attendance record will be issued by way of the FedBizOpps website only. All questions regarding this solicitation must be submitted in writing by email to Daniel.Shea2@va.gov, no later than 2:00 P.M. Seven (7) calendar days prior to the deadline for the receipt of bids. (For Example, if the Bid Deadline is May 23, 2011 questions must be received no later than 2:00 P.M. on May 16, 2011.) NO TELEPHONE REQUEST WILL BE HONORED. All Questions must be submitted utilizing the following SUBJECT TITLE: S05 RFI 503-11-119 followed by the Subject Matter and Firm's Name. For example: S05 RFI / 503-11-119 Lighting Fixtures / ABC Company. All responses to questions will be posted to the web site. The Solicitation Issue Date will be on or about August 30, 2011. Interested parties are strongly encouraged to review the entire Solicitation, especially the Specifications and Drawings prior to the Pre-Bid Conference/Site Visit. See solicitation for the Bid Due Date and Pre-Bid Site Visit. 1.Bids shall be submitted in sealed envelopes or packages (Telegraphic Bids, Facsimile bids and Bids submitted by electronic commerce are NOT acceptable.) 2.All bids sent before the time set for the opening of bids shall be mailed to the following designated official and address referenced in the Solicitation: The bids shall not be opened or viewed, and shall remain in a locked drawer and retrieved by the Designated Government Official prior to the Bid Opening. A firm's submission must include an insert detailing the following information placed in the front of the Submission. "An insert detailing the following 3 items: oDuns & Bradstreet Number oTax ID Number oThe E-mail address of the firm's Primary Point of Contact. "A copy of the letter from the Center for Veterans Enterprise (CVE) verifying the firm's status as an SDVOSB. The letter must be must be current and no more than one year before the submission deadline. And / Or A printed copy of the firms VetBiz Registry from the website.
