Description
LIKO Brand Ceiling-mounted Patient Lifts RFQ No. VA-248-11-RQ-1064 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). This solicitation document and all incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. This is a brand name specific requirement for LIKO ceiling-mounted patient lifts and only offers of LIKO brand lifts shall be considered. See attached brand name justification. The relevant NAICS code is 423450. Offers are due on August 19, 2011 at 3 p.m. Eastern Time. Offers, as well as any questions or requests for clarifications, should be submitted to Eric Sullivan, Contracting Officer at the West Palm Beach VA Medical Center, by email (eric.sullivan2@va.gov) or facsimile (561-422-6507). The deadline for submitting any questions or request for clarifications is Wednesday August 17, 2011 at 5 p.m. Eastern Time. Other means of offer submission must be submitted for consideration to the Contracting Officer who can be reached by phone, (561) 422-1272, or email, eric.sullivan2@va.gov. In all cases, offerors are strongly encouraged to confirm receipt of offers. The West Palm Beach VA Medical Center (7305 North Military Trail, West Palm Beach, FL 33410) has a requirement for LIKO brand ceiling-mounted patient lifts. All work shall be done in accordance with the enclosed specifications and attached drawings. Contractor shall provide all the necessary labor, supervision, tools, supplies/materials and equipment necessary to install 26 LIKO brand ceiling-mounted patient lifts at the VA Medical Center West Palm Beach, 7305 North Military Trail, West Palm Beach FL. The VA intends to establish a firm fixed price contract with the successful offeror. Installation is to be completed within 45 days after receipt of award. See attached provisions and clauses applicable to this solicitation and the resulting contract. Award shall be made to the responsive, responsible offeror providing the lowest-priced, technically acceptable offer. Successful offers must be registered with the Central Contractor Registration (https://www.bpn.gov/ccr/default.aspx) and provide representations and certifications either electronically online (https://orca.bpn.gov/) or in writing by requesting a written form (FAR 52.212-3 Offeror Representations and Certifications - Commercial Items) from the Contracting Officer. The Davis Bacon Act and Department of Labor Wage Determination FL100131 applies to the installation of the ceiling lifts. The Wage Determination is attached and can be accessed at http://www.wdol.gov/. Offer Submission: each offer should include the following. Additional information is not requested and is likely to be discarded without review. 1) Price per room and a total price for all rooms including installation 2) Warranty terms 3) Training included with offer Proposal Evaluation Criteria: Proposals shall be evaluated in accordance with FAR 52.212-2 Evaluation-Commercial Items as follows. (a)The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation is determined to be the lowest-priced technically acceptable offer. The VA intends to award without discussions. As such, Offerors are advised to submit all information requested and to provide their best possible offer. (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. CEILING LIFTS PROJECT SPECIFICATIONS VA MEDICAL CENTER WEST PALM BEACH The purpose of these specifications is to outline the type of patient lift equipment required and define the extent of the design and installation work needed to complete the project described herein. Anything not specifically included in this description is excluded from the agreed upon scope of work. In the event of a conflict between the work described in these specifications and the supplemental documents, this work scope definition shall govern. The contractor will be ready to begin work within 15 calendar days of the notice to proceed, and the contractor will have 90 calendar days to complete the project. Track configurations do not need to include a method to transport patients from bed to toilet. WORK INCLUDES, BUT IS NOT LIMITED TO: 1.Maintain a current copy of site-specific drawings on site and redline any deviations. 2.Provide to VA any operation/maintenance manuals, and warranty information relating to contractor-provided equipment at the end of the project. 3.Upon request, conduct weekly progress review meetings with appropriate staff members and publish minutes. 4.Coordinate all noisy or disruptive work with VA's COTR. 5.Follow all site infection control and dust abatement procedures. 6.Keep site clean; remove debris from site to VA's dumpster on a daily basis. 7.Perform construction cleaning of the site upon completion of the work. VA will be responsible for final cleaning of the room. 8.Store, stage and deliver materials to job site as required; coordinate with COTR. 9.Comply and direct contractors to comply with VA's security regulations and dress code requirements. 10.Review Statement of Work with VA representatives prior to installation to confirm Scope of Work. Review and confirm schedule. Document meeting. 11.Provide on-site supervision of the work as required by the ongoing construction. 12.Provide all required submittals including work plan, life safety plan, infection control plan and fire safety plan. 13.In conjunction with VA, develop and document procedures for construction phase portion of the work. 14.Work will be constructed in compliance with building code, VA requirements, and Vendor equipment specifications. 15.Contractor will follow all manufacturer specifications for installation of overhead patient lifts. 16.Contractor will inspect and weight test, per manufacturer guidelines, each overhead patient lift immediately upon completion of installation. Inspection of system shall include verification that all bolts are tightened; all end stops and end stop bolts are properly installed and tightened. 17.Contractor and COTR must sign documentation verifying that the installation has been inspected and that the unit has been appropriately weight tested. These documents shall be submitted to the COTR. 18.Contractor shall provide training on the use of each lift as installation in each room is accomplished. Structure: 1.Provide all hardware for securing track system to concrete structural deck. The main building in VA West Palm Beach is built with interstitial spaces above the ceiling. The structural component (not the suspended floor/ceiling) must be used as the anchor point. The facility's concrete deck has 4" of structural concrete and about 3" of topping. Overall at least 6" on average due to variability in pouring slabs. The distance between structural decks is 21' in the basement, 20' on the first floor, and 19'6" everywhere above. Ceiling height is at 9' for the drop ceiling. 2.A site visit of each room is mandatory before commencement of work and acceptance of proposal drawings. Sprinkler: Sprinklers will be relocated as necessary by the VA. Relocation of sprinkler heads is a complicated procedure. Contractor shall provide one week notice to the COTR, and confirm COTR's receipt and acknowledgement of this notification, for all fire protection sprinkler heads to be relocated. The necessary system shutdowns for the relocations shall be minimized. Mechanical/Plumbing: All supply and return air diffusers will be relocated by the VA as necessary. Contractor shall provide one week notice for all necessary relocations and confirm COTR's receipt and acknowledgement of this notification. Electrical: The VA will supply 120 Volt duplex outlets as required for equipment installation. Contractor shall specify locations, and shall allow one week for VA's work and confirm COTR's receipt and acknowledgement of this notification. Additional requirements: 1.VA will not receive, unload, transport, or load contractor's materials. 2.All work shall be executed Monday through Friday 7:00 a.m. to 5:00 p.m. Work on Saturday, Sunday and federal holidays would be at the contractor's request and convenience, it would not be cause for changes to the contract, and it must be scheduled in coordination with VA's COTR. 3.Contractor must provide for its own secure storage. 4.Parking is extremely limited. Contractor shall park in lots 18 / 19. 5.As a result of the work location and its proximity to VA patients, tools and equipment shall not be left unattended. All tools and equipment shall be in direct line of sight of the contractor at all times. 6.Contractor's workmen are subject to all the rules of the medical center. 7.Contractor and all on-site workers must attend onsite Safety and Infection control orientations. 8.Security clearance is required. Contractor shall provide 2 photo ID's and subsequently obtain VA issued ID card.
