- This is NOT a solicitation announcement. This is a request for information only. The purpose of this sources sought synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled Veteran-Owned, Veteran-Owned Small Business, Hubzone, 8(a), small, small disadvantage and large business) relative to NAICS code 562219 (size standard of $12.5 millions). Responses to this request for information synopsis will be used by the government to make appropriate Acquisition decisions. Responses to this sources sought shall NEITHER constitute responses to solicitation which is not currently available NOR a request to be added to a prospective bidders list to receive a copy of the solicitation. The decision to solicit for a contract shall be solely the Government's discretion. The Michael E. DeBakey VA Medical Center (MEDVAMC) is seeking information on a potential contractor to provide all labor, materials, parts, equipment, transportation, documentation, and supervision to pick up, treat, and dispose about 45,800 lbs per month of infectious medical, pathological and chemotherapy wastes from its main hospital and outpatient clinics located in the following areas: "Main hospital-MEDVAMC: 2002 Holcombe Boulevard, Houston, TX 77030 "Beaumont VA Community-Based Outpatient Clinic (BOPC), 3420 Veterans Circle, Beaumont, Texas 77707-2552 "Charles Wilson VA Community-Based Outpatient Clinic (CWOPC), 2206 North John Redditt Drive, Lufkin, Texas 75904 "Galveston VA Community-Based Outpatient Clinic, 3828 Avenue N, Galveston, TX 77550 "Conroe VA Community-Based Outpatient Clinic (COPC), 800 Riverwood Court, Conroe, Texas 77304 "Richmond VA Community-Based Outpatient Clinic (ROPC), 22001 Southwest Freeway, Suite 200, Richmond, Texas 77269 "Lake Jackson VA Community-Based Outpatient Clinic, 208 South Oak Drive, Suites 700, Lake Jackson, 77566 Contractor will be required to provide the following quantity of containers to be placed onsite at the above hospital and outpatient clinics for daily collection, storage, and transport of infectious medical, pathological and chemotherapy wastes: "Main hospital: a minimum of 100 each "new" 97-100 gallon, portable pre-labeled biohazard closeable lid containers for the daily collection, storage and transport of infectious medical waste and 20 each for chemotherapy waste disposal. Containers shall be washed and sanitized daily before delivery and reuse. Also, containers shall be maintained in a as manufactured-condition or replaced at Contractor's expense, as needed in order to meet OSHA and DOT regulations. "All outpatient clinics: a minimum of 7-10, 28-30 gallon per-cleaned per-labeled biohazard closeable-lid storage containers at EACH clinic. Each container shall be maintained and replaced at Contractor's expense, as needed to meet OSHA and DOT regulations. Containers shall be washed and sanitized daily before delivery and reuse. Also, containers shall be maintained in a as manufactured-condition or replaced, in order to meet OSHA and DOT regulations. Contractor will be required to perform all work in accordance with the specific services requirements for each facility identified in the Statement of Work (SOW) and approved treatment methods enforced by federal, state, and local laws, regulations, and ordinances, including the TCEQ, TDH, OSHA, DOT, CDC, and EPA. If the Government decides to solicit for a contract for the aforementioned services, the Government contemplates a month-to-month contract for a base term of 6 months with a one 3-month option for renewal. Sources interested in this notice shall submit the following information: 1.Company name, address, point of contact, cage code, size of business pursuant to North American industrial classification system (NAICS) 562219. 2.Evidences of applicable business permits, licenses required by the Federal, State, and Local solid waste regulations to provide the aforementioned services in ALL geographic areas described above. 3.Evidences of capability to provide the aforementioned services. These evidences shall include but not limited to the list of existing customers having contracts for receiving the said services from contractor. 4.and responses to the following questions: (1)Does your company have capability to provide the pickup, treatment, and disposal of infectious medical, pathological and chemotherapy wastes to ALL geographic areas, the main hospital and all of its outpatient clinics listed above? (2)Is your firm registered in Central Contractor Registration: www.ccr.gov. (3)Is your business large or small business under NAICS 562219? (4)Is your firm a certified service-disabled veteran owned and veteran-owned small business? If yes, is your firm registered at http://www.vip.vetbiz.gov/general_user/search/default.asp and verified by the VA's Center for Veterans Enterprise (CVE)? (5)If small, is your firm qualified as a small, emergent business, or a small, disadvantaged business? (6)If disadvantaged, specify under which disadvantaged group and is your firm certified under section 8(a) of the small business act? (7)Is your firm a certified "hub zone" firm? (8)Is your firm a woman-owned or operated business? This is not a request for proposals or request for quotes and in no way obligates the government to award any contract. Responses to this notice shall be in writing by e-mail addressed to Ms. Nam-Phuong Nguyen at Namphuong.nguyen@va.gov. All responses must be received in writing no later than 4:30 PM, local time on August 22, 2011. Telephone responses will NOT be accepted. Firms responding to this announcement, which fail to provide all of the required information, cannot be used to help the government make the acquisition decisions, which is the intent of this synopsis. This notice is to help the VA medical center in determining potential sources only. No solicitation is currently available. All firms responding to this request for information notice are advised that their response to this notice is not a request to be considered for a contract award or to be placed on the Government's prospective bidders list to receive a copy of the solicitation.
