- ATTACHMENT 3: Bid Schedule ATTACHMENT 2: Justification & Approval Letter ATTACHMENT 1: Statement of Work (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The government intends to solicit and negotiate with only IZASA PORTUGAL under authority FAR PART 6.302-1. Acquired services consist of providing preventive maintenance and inspection s for the ACT-T 5 DIFF CP, SERIAL #A148253 medical equipment. IZASA PORUGAL has been found to be the only vendor authorized to provide these services on existing and already installed equipment. (ii) The solicitation number is F2N4MS1167A001; this solicitation is a Request for Quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-53. (iv) This acquisition will not be full and open competition; the North American Industry Classification System (NAICS) code is 811219. The business size standard is $19.0. (v) See Attachments (vi) Attachment 1 - Statement of Work (SOW) Attachment 2 - Justification for Other than Full and Open Competition Attachment 3 - Bid Schedule (vii) Place of Performance: Lajes Field, Azores, Portugal, this contract will start 1 Oct 2011.
