- Badge and Badge Holder Specifications This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals; quotes are being requested and a -written solicitation will not be issued. The solicitation number is DTMC-75-11-Q-00085 and it is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. This acquisition shall be a 100% Small Business Set-A-Side. The NAICS Code is 316993 and the Small Business dollar standard is 500 employees. The Contract Line Items to be acquired are as follows: BASE YEAR CLIN Description QTY Unit Price Total Price 0001 Badges 800 0002 Badge Holders 800 OPTIONAL LINE ITEMS 0003 Badges 0-100 0004 Badge Holders 0-100 Option Year I 1001 Badges 0-100 1002 Badge Holders 0-100 Option Year II 2001 Badges 0-100 2002 Badge Holders 0-100 Option Year III 3001 Badges 0-100 3002 Badge Holders 0-100 Option Year IV 4001 Badges 0-100 4002 Badge Holders 0-100 This requirement will be awarded as a Firm Fixed Price Contract with a twelve month Base Period and four twelve month option periods. The Quantities cited under CLINS 0003; 0004 and 1001 through 4002 represent the Government's best estimated of quantities it may order during the life of the contract and should not be construed as a minimum guarantee. The contractor shall be required to provide the requested supplies in accordance with Attachment 1 Product Specifications. Delivery of the requested items must FOB Destination and delivery no later than 45 days after contract award. The following Federal Acquisition Regulations (FAR) provisions at 52.212-2, Evaluation of Commercials applies to this acquisition. The following evaluation factors will be used for the award of this acquisition listed in descending order of importance: (1) clearly demonstrated the Offerors process and procedures to secure and minimize loss and theft throughout the entire supply change. All losses must be report to FMCSA immediately (2) Delivery Schedule (3) Offerors must submit 3 the name address and telephone number of three Past Performance references from customers that have purchased the same or similar product and (4) Price. Offerors must include a completed copy of the provision at 52.212-3, Offerors Representations and Certifications-Commercial Items with its offer. FAR Clause 52.212-4 Contract Terms and Conditions -Commercial Items applies to this acquisition. FAR written notice of award or acceptance of an offer mailed or otherwise furnished to the successful Offeror within the time for acceptance specified in the offer shall result in a binding contract without further action by either party. Clause 52.212-4, Contract Terms and Conditions - Commercial Items, is incorporated by reference into the solicitation. Finally, the Transportation Acquisition Regulation (TAR) clauses 1252.217-74 - Subcontracts; 1252.237-70. Quote are due no later than 2:00 p.m. on August 25, 2011. Offeror quote shall be a maximum of 30 pages. Offerors must submit via proposal via mail or designated carrier and electronically to melinda.riddick@dot.gov. However, it is each offeror's responsibility to ensure the offer is received (in total) by the Contract Specialist prior to the deadline.
