- Amendment 001 Attachment 6 - Maps 4 of 4 Attachment 6 - Maps 3 of 4 Attachment 6 - Maps 2 of 4 Attachment 6 - Maps 1 of 4 Attachment 5 - Forest 2011 ERFO Repair Project List Attachment 4 - Sampe Damage Survey Report Form Attachment 3 damage Survey Report Form Request for quotes, specifications, and attachments 1 and 2 The Forest Service is seeking quotes for the following: Contractor shall furnish all technical expertise, labor, equipment, supervision, transportation, operating supplies and incidentals to complete a Damage Survey Report (DSR) package for each site identified in the attached tables. The contractor shall determine a proposed repair for the site except for those noted for coordination with local Zone Engineer. Each DSR will contain the following information: Itemized cost estimate and description of the proposed repair, Map (8 ½ x 11) of the damaged site (1" = 1 miles scale), Sketch of damage (8 ½ x 11) with sufficient dimensions to determine volumes, Sketch of proposed repair (8 ½ x 11), and Photos documenting damage (Color - 2 minimum). Contractor shall have 10 years minimum experience designing and or cost estimating USDA Forest Service type construction projects, with the capability to survey and measure road features and the math skills to determine volumes and quantities. The contractor must be familiar and show through their past experience knowledge of Forest Service Specifications and Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects (FP-03). Contractor is not required to be a Licensed Professional Engineer. The contractor must also demonstrate good writing and communication skills.
