Description

This is an announcement from the Michael E. DeBakey VA Medical Center (MEDVAMC) prior to the issuance of a solicitation. The MEDVAMC hereby announces its intention to solicit a Firm Fixed Price, Indefinite Quantity contract with a responsible and qualified contractor who shall provide all labor, materials, equipment, transportation, and supervision to deliver infectious medical and chemotherapy waste treatment and disposal services to MEDVAMC, 2002 Holcombe Blvd., Houston, TX 77030, and its seven community-based outpatient clinics (CBOC) in accordance with an approved treatment method enforced by federal, state, and local laws, regulations, and ordinances, including the TCEQ, TDH, OSHA, DOT, CDC, and EPA. The term of contract will be a month-to-month basis for about six months. The request for quote (RFQ) is assigned reference number VA-256-11-RQ-0859 and is set aside 100% for service-disabled veteran-owned small business. The NAICS code 562219 and its small business size standard of $12.5 million dollars apply to this solicitation. The RFQ will be posted and available for download on September 2, 2011 from https://www.fbo.gov/. Quotes will be due back in the issuing office on September 19, 2011, 4:30 PM (U.S. Central Time). Interested offerors are solely responsible for monitoring website www.fbo.gov for amendment(s) to the RFQ if any and acknowledging them in accordance with the requirements stipulated in the amendments. All questions related to this announcement and the RFQ must be in writing and emailed to contracting officer, Ms. Nam-Phuong Nguyen at Namphuong.nguyen@va.gov. Faxed and phone inquiries and faxed quotes are NOT allowed. All responsible sources may submit a quote that will be considered by the agency. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. As stipulated in Veterans Affairs Acquisition Regulation VAAR 852.219-10, part of the definition for a small business concern to be considered a service-disabled veteran-owned small business concern is that the business has been verified for ownership and control and is so listed in the Vendor Information Pages database, (http://www.VetBiz.gov).
