Description: AMENDMENT #4: The purpose of this amendment is as follows: 1)respond to offerors' questions 2)replace page 13 (price/cost/schedule); insert the price/cost/schedule page provided in this solicitation. 3)replace page 19 (X-ray Inventory List) insert the X-ray Inventory List page provided in this solicitation. The closing date for this solicitation is September 13, 2011/11:00 AM (EST). All responses to questions will be posted by way of amendment to this combined synopsis/solicitation to the FedBizOpps website (The email address for all questions to this combined Synopsis/Solicitation is rosa.rivera3@va.gov). QUESTIONS Question #1: Are the General x-ray/fluoroscopy units one or two tubes? Typically they contain two tubes. If two, then there are many more than 21 tubeheads to be tested. Government Response: All of the General x-ray/fluoroscopy units have one tube, except line items 5 (Siemens Luminos), 6 (GE Precision 500), 16 (Toshiba Kalare), 17 (Philips Easy Diagnost), 18 (Philips Easy Diagnost) which have two tubes. With that being said, there are 22 x-ray tubes, 25 x-ray tubes on the dental, 3 x-ray tubes for the portables, 3 x-ray tubes for the c-arms, and 3 tubes for the bone-dexas. The QTY for Line Item 29 has been changed to 3 (2 Orlando and 1 Viera) because the GE/OEC 9900 (Line Item 20) is considered a stationary c-arm. The total amount of tubes is 56. Question #2: Are the CT/Mammo/Cameras included in the 21 total tubeheads? If so, then how do the 3 portable x-ray units, 4 portable c-arm units and 3 bone densitometer units fit in this equation? They also would bring the total up to more than 21. Government Response: CT and Mammo are included in the 22, but the Cameras do not have tube heads. Cameras have dual heads. There is a total of 4 dual head cameras. Question #3: There is no place on the price/cost schedule to price out either the CT/Mammo/Cameras or the extra x-ray/densitometer units if they are the ones not included in the 21 total.
