Solicitation Number: VA-249-11-RQ-0744 Notice Type: Sources Sought This is a SOURCES SOUGHT SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA-249-11-RQ-0744 and is issued as a Request for Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-35, July 14, 2009. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. (v) This requirement consists of one (1) line item: Closed Mattress Systems for Behavioral Health wards- (Qty - 20 ea) (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: Closed Mattress System for Behavioral Health Wards VISN 9 Contracting is requesting a quote for a one-time, firm-fixed price purchase of (20 EA) Closed Mattress Systems for Behavioral Health patients at the VA Medical Center, Murfreesboro, Tennessee that meets or exceeds the specifications listed below. The closing date for this RFQ is September 14, 2011 at 12:00 PM CST. The mattresses will be shipping to Alvin C York VA Medical Center, 3400 Lebanon Road, Murfreesboro, Tennessee 37129. Please enclose the following information in your quote. 1.Mattress type with full specifications that meet or exceed the specifications listed below (include brochures if available) 2.Firm-fixed pricing, quantity of 20 ea, for the closed mattress systems 3.Delivery type: F.O.B. Destination 4.Estimated Delivery Timeframes 5.Warranty information Closed Mattress System's Salient Characteristics Mattress Dimensions: Size - 36"x 80" x 6.5" Mattress Material Requirements: "Top Cover: oClosed mattress system (one piece cover, no zippers, no overlap seems, no vents) oNo Chlorofluorocarbons: an organic compound that contains carbon, chlorine, and fluorine, produced as a volatile derivative of methane and ethane. oLatex free oFree of plastic or vinyl that could be removed and used for suffocation. oNon-plasticizing; (will never become brittle or crack) oAntibacterial "Mattress that provides an impermeable barrier to microbial agents commonly involved in nosocomial infections oTreated to meet or exceed Fire Code (para 10.3.2.2 and para 10.3.4 of NFPA 101, Life Safety Code) ofabric which is flexible (for prevention of pressure sores) and has a low MVT (for barrier protection of the core "MVT Moisture Vapor Transmission is the passage of molecules of moisture vapor through pores or openings of a permeable or semipermeable material. Pillows and mattresses should not have non-breathable covers that can be removed by the patient and used for suffocation. This requirement is consistent with eliminating plastic trash can liners (refer to question #35) and vinyl or plastic shower curtains (refer to question #59).
