Description

This document is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested, and a written solicitation will not be issued. The proposed contract action is for supplies or services for which the Government intends to solicit competitive offers. Interested persons may submit offers, and all eligible offers received prior to Sept 15, 2011 will be considered by the Government. The NAICS code is 541511 and the size standard is $25.0 Million. For example, the software must be able to pre-define a response field as taking a number between 1 and 3, or one of the letters, a, b, c (whether capitalized or not), or a time such as 12:00:00 8.The reader subsystem should accommodate bar codes 9.The reader subsystem should include an efficient subsystem for the verification of all responses. 10.The reader subsystem must include an alarm utility that will alert the user to patient responses that it cannot interpret automatically, and an efficient utility for user "correction" of those responses. 11.Software must include an export subsystem that interfaces seamlessly with Microsoft Access and Excel such that the scanning of a paper form automatically results in the export of all patient-entered data into specified variables in a specified database or spreadsheet (see 6.) 12.Software installation model must accommodate multiple users, but software can reside on a single workstation INSTRUCTIONS TO OFFERORS Only electronic offers will be accepted. EVALUATION FACTORS Offerors will be evaluated to the extent they can provide the items (or equal) listed in the price schedule in Section B. The Government will award to the lowest priced, responsible, technically acceptable offeror. FAR 52.211-6 Brand Name or Equal (Aug 1999) (a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. (b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must- (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by- (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. (c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation.
