i.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. ii.This is a request for quotes (RFQ) and is referenced by solicitation number VA-263-11-RQ-0862. This solicitation is for a firm fixed price purchase order. iii.The solicitation document, and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-53. iv.In accordance with FAR part 19 - Small Business Programs, this solicitation is a total small business set-aside. Vendors offering products other than the Audioscan Verifit Real Ear Hearing Aid Analyzer shall demonstrate that their items perform all the same functions, have the same dimensions and conform to the same construction and safety standards of the specified items. Wireless Bluetooth patient care modalities must be compatible with NOAHLink hearing aid programming systems. Systems not compatible with current VA audiology-related modalities will be determined unsuitable for VA use. New equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's original warranty. CAUTION to OFFERORS. The purchasing activity is not responsible for locating or securing any information that is not identified in the RFQ or reasonably available to the purchasing activity. vii.Delivery Schedule: a.Freight on Board (FOB) Destination. FOB Destination is preferred, please include any freight in the individual item pricing if possible. The government will arrange for shipping via VCBL if the estimated cost for handling is over $250.00.
