- Clauses & Provisions Mod 1 This is a Modification to the orginial Solicitation number BPD-AFRG-12-CI-0001 We are Modifing the RFQ to change the requirement on the Period of Performanc. and to extend the Response Date. The new Period of Performance will be an award for a 12 month Base period with four (4) 12 month options as follows: Base: October 1, 2011 - September 30, 2012 Option Period I: October 1, 2012 - September 30, 2013 Option Period II: October 1, 2013 - September 30, 2014 Option Period III: October 1, 2014 - September 30, 2015 Option Period IV: October 1, 2012 - September 30, 2013 All other requirement remain unchanged. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation BPD-AFRG-12-CI-0001-1 is issued as a Request for Proposal (RFP). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-47. The NAICS code is 711130 and the size standard is $7 million. This is an 100% Small Business Set-A-Side procurement. The Bureau of the Public Debt on behalf of the US Armed Forces Retirement Home-Gulfport MS, has a requirement for pianist/musical Services locate at the Armed Forces Retirment Home, 1800 Beach Drive, Gulfport, MS 39507 in accordance with the attached Performance Work Statement. Period of Performance: Base plus Four Option Periods. Services to be performed at AFRH-Gulfport location. The provisions at 52.212-1, Instructions to Offerors - Commercial Items (JUN 2008), applies to this acquisition. Addenda to this provision are attached. The provisions at 52.212-2, Evaluation - Commercial Items (JAN 1999), applies to this acquisition. The following factors shall be used to evaluate offers: Technical Capabilities, Past Performance, Price. Addenda to this provision are attached. Offerors are to include a completed copy of 52.212-3, Offeror Representations and Certifications - Commercial items (OCT 2010), with their offer. 52.212-4, Contract Terms and Conditions - Commercial Items (JUNE 2010) applies to this acquisition; Addenda to 52.212-4 are attached. Supplimental clauses apply to this acquisition: Defense Priorities and Allocations Systems (DPAS) and assigned rating are not applicable to this acquisition.
