Description

Synopsis: The Department of Veterans Affairs, Albany VA Medical Center, Albany, NY intends to negotiate on a sole source basis (iaw FAR 13.106-1) for GS-UE160-AL5 radial array video gastroscope system from FSS, V797P-4282A, Olympus America, 3500 Corporate Pkwy, Center Valley, PA 18034. The statutory authority permitting other than full and open competion is FAR 8.405-6 (b)(1). Only one source is capable of responding due to the unique or specialized nature of the equipment. In addition, Open Market materials listed below are required. Interested parties may identify their experience in providing the additional open market materials and services to complete the procurement of the Evis Exera II Video System to the Contracting Officer via e-mail at richard.williams18@va.gov or fax (315) 425-4883. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA-528-11-RQ-0367 and issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43, effective August 2, 2010. The associated NAICS code is 339112 and small business size standard is 500 employees. The acquisition is not set-aside for small business. The requirement for the Evis Exera II Video System includes the following Open Market Materials: ITEM QTY UNIT UNIT COST TOTAL COST 1. Product Number: MAJ-1597 Detachable ultrasound cable for use with GC-UCT180 and BF-UC180F for compatibility with the EU-ME1 and select Aloka Prosound processors. 1 Each $_________ $________ The date of delivery is ordered by purchase order with delivery being FOB Destination to the Department of Veterans Affairs, Albany VAMC, 113 Holland Ave., Albany, NY 12208-3410.
