Solicitation Number: VA-241-11-RQ-1341 Notice Type: Combined Synopsis/Solicitation Synopsis: (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA-241-11-RQ 1341 and is issued as an RFQ. (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-52, May 31, 2011. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) VISN 1 Contracting Activity intends to negotiate a sole source contract to provide services for provision of urgent street level psychiatric evaluation, substance abuse crisis intervention and consultation, and links to transitional housing, peer support and assistance with HUD-VASH vouchers application process or homeless veterans. Under the authority of FAR Part 6.305-5 In accordance with FAR 6.305, the Justification which further identifies the urgency will be posted to FBO within 30 days following the award. However, all other responsible sources that feel they can provide this item may submit a capability statement, proposal, or quotation, which shall be considered by the agency. The North American Industry Classification System (NAICS) code is 624229, and the size standard is $ 5,000.000.. Description of requirements: The Contractor shall work with the National Center on Homelessness among Veterans to incorporate Peer Support, Substance Use Disorders specialist(s), and, if necessary, medical personnel, into an existing program of mobile case management that specifically targets homeless Veterans, provides physical and psychological intake assessments, immediately connects the Veteran with options for a transitional housing arrangement, links Veterans with mental health and primary care, and uses Peer Support Specialists to work with HUD-VASH housing specialists and clinical case managers to obtain permanent housing vouchers and residences for eligible Veterans.
