Description
The Health Care for Homeless Veterans (HCHV) Program, located at the North Texas Veterans Health Care System, 4500 S. Lancaster Road, Dallas, Texas, has a requirement to assist with the provision of residential placement and supportive services within the rural areas of Tyler, Texas. At the Contractor's facility, the Contractor shall serve homeless Veterans in need of immediate placement in a safe, transitional housing environment. Services shall be in accordance with the Statement of Work of the contract. Contractor will be responsible for providing sufficient residential capacity to place up to 20 Veterans in safe community-based residences. Suitable residential placement generally requires: "Staffing available on-site 24 hours a day, 7 days a week "Staffing in reference to security, available on-site 24 hours a day, 7 days a week, to ensure proper placement of residents on and off the facility location "Three (3) nutritionally adequate meals a day, 7 days a week "Laundry facilities and detergent available for Veteran to do laundry "The ability to place a Veteran on the same day of a referral from HCHV staff or Contracting Officer Technical Representative (COTR) "Hygiene products including wash cloths, soap, feminine products, deodorant, toothpaste, and tooth brush shall be made available to Veterans. "Transportation to VA Medical appointments or vouchers for use of public transit. "Suitable bedroom space and furnishings allowing for adequate storage and security of belongings, with no more than 2 Veterans per room less than 140 square feet. Services which the Contractor must be able to furnish include: "Structured group activities as appropriate. Examples include group therapy, social skills training, Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and physical activities. "Collaboration with appropriate VA staff to ensure linkages to and provision of supportive psychosocial services. "Assistance to develop responsible living patterns and to achieve a more adaptive level of psychosocial functioning, improved social skills and improved personal relationships. "Support for an alcohol/drug abuse-free lifestyle, with regular witnessed drug and alcohol screening "Assistance to gain and apply knowledge of the illness/recovery process. "Individual professional counseling, including counseling on self care skills, adaptive coping skills and as appropriate vocational rehabilitation counseling in collaboration VA program and community resources. "Specialized individual and group support addressing the needs of frail, elderly Veterans. "Specialized individual and group support addressing the needs of Veterans with complex medical and mental health diagnoses. "Contractor shall possess the ability to clinically assess and respond to a Veteran in a mental health crisis and will create an environment conducive to medical and mental health recovery as needed. The anticipated Period of Performance for these services is October 1, 2011 through September 30, 2012. The Request for Proposal (RFP) number is VA-257-11-RP-0232. The applicable NAICS code is 624229 and the small business size standard is $7.0 million. The estimated issue date is September 15, 2011, and the anticipated due date September 23, 2011 at 3:00:00 PM Central Standard Time.
