Description
The proposed contract action is for commercial item, which the Naval Medical Center San Diego intends to solicit this requirement on a sole source basis to Central Admixture Pharmacy Services, Inc., under the authority of Far 13.106-1 (b). This is a combined solicitation/synopsis for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-10-T-0669 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 541511. All interested bidders shall submit quotations electronically by email to reynaldo.devera@med.navy.mil or by facsimile at 619-532-5596, attention Reynaldo de Vera. E-mail submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions. Quotations are due on or before 30 September 2011 at 07:00 A.M. Pacific Standard Time to be considered responsive. Annual Service Agreement for preparation of Total Parenteral (TPN) solutions and delivery for Naval Medical Center San Diego to be provided by the contractor in compliance with JCAHO USP Chapter 797. Period of Performance: 01 October 2011 thru 30 September 2012 CLIN 0001 Total Parenteral Nutrition Bags Qty 2424 Each $_______ CLIN 0002 Standard Stock Bags Qty 192 Each $_______ CLIN 0003 Neonatal Lipid Syringes Qty 720 Each $_______ Under the authority of FAR 13.106-1(b), sole source justification is based on Central Admixture Pharmacy Services, Inc., being the sole provider for preparation of Total Parenteral Nutrition solutions which can truly satisfy Government minimum requirement. CAPS is the only vendor that provides service within close proximity of NMCSD that can meet the Inpatient Pharmacy needs. Contractor must be registered to the Central Contractor Registration (CCR) prior to award. AVAILABILITY OF FISCAL YEAR 2012 FUNDS. THE GOVERNMENT OBLIGATION MADE HEREUNDER IS CONTINGENT UPON THE ENACTMENT OF THE FISCAL YEAR 2012 APPROPRIATION ACT. THIS ORDER IS ISSUED SUBJECT TO THE 2012 DOD APPROPRIATION ACT OR AN EXTENDED FISCAL YEAR 2011 CONTINUING RESOLUTION ACT AND IS SUBJECT TO ALL PROVISIONS OF WHICHEVER ACT BECOMES APPLICABLE. THE CLAUSE 52.232-18 REFERENCED HEREIN APPLIES TO THIS CONTRACT. STATEMENT OF WORK GENERAL REQUIREMENTS: The effort required hereunder shall be performed in accordance with this Statement of Work and in accordance with all other terms and conditions set forth herein. The contractor shall provide all services, materials and equipment necessary for preparation of Total Parenteral Nutrition (TPN) solutions, as described in the Attachment A, and its delivery services to Pharmacy Department, Naval Medical Center San Diego. The scope of work performed under these specifications includes the furnishing of all labor, and parts to perform all services listed to assure continued operation at their designed efficiency and capacity. LIABILITY: The contractor shall not be liable for any loss, damage, or delay due to any cause beyond his reasonable control including but not limited to, acts of government, strikes, lockouts, fire, explosion, theft, floods, riot, civil commotion, war, malicious mischief or acts of God. UTILITIES: The contractor may use Government utilities, (e.g., electrical power, compressed air, and water) that are available and required for any service performed under this contract. The building engineer, to ensure compatibility with the Naval Medical Center's electrical wiring and equipment, must approve contractor electrical equipment. CONTRACTOR CHECK-IN/CHECK-OUT: The contractor, or his representative, is required to report to the Medical Repair Branch for Visitor Badges during the hours of 7:30am “ 8:00pm, Monday through Friday, prior to and upon completion of any service/repair performed. PARTS AVAILABILITY: To ensure minimal equipment downtime, the contractor shall maintain replacement repair parts and materials within 7 days. COMPENSATION: Labor: All compensation for labor is included in the contract price. Parts and materials: All compensation for parts and materials is included in contract price. Parts/Supplies Quality: Parts and Supplies provided under this contract shall be guaranteed to be equal in all respects, including performance, interchangeability, durability, and quality to the OEM parts when new or as presently recommended by the manufacturer.
