- The purpose of this amendment is to: 1) clarify how proposals are to be submitted and 2) change the requirements in Section 1.4 "Quality Control" of the Statement of Work. 1. The offerors' proposal shall consist of two separate volumes: Volume I, Technical, and Volume II, Price. Offerors shall provide one hard copy and three CD's of the proposal. The files on the CD must match exactly with the hard copy proposals. The submission must be clearly indexed and logically assembled. All files must either be in PDF format or in a format compatible with Microsoft Office 2003. 2. Section 1.4 "Quality Control" of the Statement of Work is hereby deleted in its entirety and replaced with the following Section 1.4 "Quality Control": 1.4 QUALITY CONTROL The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor's quality control program is the means by which he assures himself that his work complies with the requirement of the contract. The Quality Control Plan is to be delivered with the contractors' proposal. After acceptance of the quality control plan the contractor shall receive the contracting officer's acceptance in writing of any proposed change to his QC system. MSHA may elect to perform a site visit prior to contract award, and to perform a Quality Assurance audit during the contract term. In addition to participating in the Proficiency in Analytical Testing program, the Contractor will institute and maintain an internal proficiency testing program for designated analyses requested by MSHA on a recurring basis. The proficiency testing will be performed on a quarterly basis on a set of blind samples prepared or procured by the Contractor. Method detection limits and levels of quantification will be updated quarterly, at minimum, for all methods utilized for analysis of MSHA samples. A copy of the Contractor's AIHA accreditation certificate, PAT sample and internal proficiency testing results will be furnished with Contractor's proposal. Contractors must also provide Standard Operating Procedures (SOPs) for total cyanides, hexavalent chromium, metals, mercury, organics and sulfuric acid. Each SOP shall not exceed 15 pages. Upon award, the successful offeror must provide a copy of their Quality Assurance Manual and all SOPs utilized for MSHA samples to the COTR. MSHA will also be provided copies of any SOP revisions relevant to requested analytical services that are made after the issuance of the contract, and will be notified of any changes in personnel assigned to contract work. MSHA may also request other records, at its discretion, (i.e. instrument calibration records, QC charts, training records) to validate Contractor sample data.
