Description

Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation VA-250-11-RQ-0591_1 is being issued as a Request for Quotations (RFQ) and the intent is to award a firm fixed price contract for supplies and installation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. This requirement is unrestricted-insufficient small business for commercial items under the North Atlantic Industrial Classification System (NAICS) 339112 Surgical and Medical Instrument Manufacturing, size standard in number of employees: 500. Requirements: Network Contracting Activity (NCA) 10 for the Louis Stokes Cleveland VAMC is seeking quotes for Mizuho OSI PROfx Surgical Table System or brand equivalent. Use of brand name is intended to be descriptive and not restrictive. Line items with specifications are as follows: (Clin 001) Mizuho OSI PROfx Surgical Table System. The system consists of the following components ¦.. a.Mizuho Surgical Table with Mizuho OSI Tempur-Med Pad b.Auxiliary Imaging Top c.Backup Pendant Hand Control d.Swivel Mount for Pendant e.Patient Care Kit, Prone f.Traction Boot pair, X-large g.Universal Foot Board (Includes 2 side rail adaptors) h.Auxiliary Clamp for PROfx i.Perineal Post Extender j.Well Hip Fixation Frame 8mm for PROfx k.Well Hip Fixation Frame 10mm for PROfx l.6 pc. Tempur-Med OR Pad Set m.Spar Lift Assist n.Femoral Hook Support Bracket o.Femoral Hook, Left p.Femoral Hook, Right q.Traction Lifter r.Traction Lifter Bracket s.U Handle Crank t.Bi-lateral Mounting Bracket -(Clin 002) - Installation (Staff in-service) Quotes with additional features will be considered, provided the minimum requirements of the Government have been met. Vendors should submit product information with quote demonstrating salient features of products. Each line item must have a description and pricing information. Delivery: FOB destination is preferred. The government will arrange for shipping if the estimated cost for handling is over $250. Products shall be delivered 45 ARO, to 10701 East Blvd, Cleveland, OH 44106. NOTE: The Government reserves the right to award without discussions, vendors shall submit the most favorable terms. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) and shall be submitted to Financial Management Systems, PO Box 149971, Austin, Texas 78714. In the event of conflict between a delivery order or task order and this contract, the contract shall control. (c) If mailed, a delivery order or task order is considered "issued" when the Government deposits the order in the mail. Orders may be issued orally, by facsimile, or by electronic commerce methods only if authorized in the Schedule. Award will be made on the basis of Lowest Price Technically Acceptable.
