Solicitation Number: 549-12-1-230-0001 Notice Type: Sources Sought Sources Sought Only. This Sources Sought Notice is not a request for formal proposals or quotes. No formal solicitation document exists at this time. The Department of Veterans Affairs North Texas Health Care Systems (VANTHCS), 4500 S. Lancaster Road, Dallas, Texas 75216 is conducting this Market Survey for qualified firms capable of providing twenty-four (24) hour 7 day a week ambulance service consisting of basic and advanced life support transportation to also include wheelchair van services for the Dallas/Fort Worth Metroplex area, specifically the Dallas VA Medical Center and the Fort Worth Outpatient Clinic and 4243 Polk Street Annex, Dallas Texas 75216. The Service shall include a requirement of for Basic Life Support and Advance Life Support Ambulance Services. The Government is seeking information for market research purposes only. The purpose of this sources sought is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business) relative to NAICS 621910, size standard $7M. The Government may or may not issue solicitation documents. Responses to this Sources Sought will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, Business size ( large or small) indentify whether or not firm falls into any of the socioeconomic categories and any pertinent information which demonstrates firms ability to meet the above requirement. Vendors are encouraged to provide a brief capability statement/prior work within the scope of work being requested. This not a solicitation announcement for proposal and no contract will be awarded from this announcement. Responses to this notice must be submitted in writing (email) and received no later than October 21, 2011@ 12:00 P.M CST. All correspondence and responses shall be e-mailed only to: E-MAIL: brian.delaney@va.gov Description of work to be performed: 2.Contractor Employees: (a) The Contractor shall furnish qualified personnel as required by contract specifications (see paragraph 3 below) to accomplish all services under this contract. (b) The Contractor shall be responsible for furnishing all vehicles, personnel, equipment and supplies, vehicle fuel, uniforms, nametags and/or badges for performance of services. (3)Contractor Personnel Qualifications: (a)All Contractor personnel performing contract services shall meet the qualifications as specified in this contract, as well as qualifications required by Federal, State, County and local government entities from the place in which they operate. Contractor personnel shall meet these qualifications at all times while performing contract services. (b) Emergency Medical Technician (EMT) and Paramedic Qualifications: EMT ™s and Paramedic ™s providing services under this contract shall have the following qualifications, in addition to those required by Federal, State, and Local Government: 1.Have completed training in accordance with the standards published by the Department of Health and Human Services with a minimum curriculum of 150 hours of equivalent including an in-hospital training period. Such training programs must also be acceptable under the regulating requirement for local EMS Systems supported by DFIHS under PL 93-154, Federal Register 39:24304. (1974). 2.Shall be certified, licensed or otherwise officially recognized by the local, state or regional government or public entity where the emergency ambulance service is operated or by which it is governed. (c) Registered Nurse (RN): Personnel assigned by the Contractor to perform the services covered by this contract shall be Licensed Registered Nurses in a State, Territory, or Commonwealth of the United States or the District of Columbia. The Qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the BA Facility Director or his/her designee. (d) Ambulance Driver: Drivers shall have a valid operator ™s or chauffeur ™s license in accordance with Federal, State and local government requirements for their place of operation, for the services they perform, be capable of administering oxygen and have successfully completed the Standard and Advanced First Aid Course of the American Red Cross or U.S. Bureaus of Mines or equivalent and be capable of providing necessary medical assistance to the attending medical care specialist. B.3.3PERSONNEL SECURITY: B.3.3.1 Contractor shall observe the requirements imposed on sensitive data by law, federal regulations, VA status and DM&S policy and the associated requirements to ensure appropropriate screening of all personnel because of the inherent sensitivity of data at the facilities concerned and the level of security clearance carried by the Health Care Facility (HCF) personnel for the function under contract. Contractor shall ensure that their personnel meet the above restrictions and that confidential and proprietary information shall be divulged only to those officers and officials of the VA Medical Center that are authorized to receive such information. B.3.3.2 Contractor personnel performing work under this contract shall satisfy all requirements for appropriate security eligibility in dealing with access to sensitive information and information systems belonging to or being used on behalf of the Department of Veterans Affairs. To satisfy the requirements of the Department of Veterans Affairs a low risk level Background Investigation shall be conducted prior to performing work under this contract. Appropriate Background Investigation (BI) forms will be provided upon contract award, and shall be completed and returned to the VA Office of Security and Law Enforcement (OSL&E) within 30 days for processing. Access to VA information resources cannot be made available until the Security Investigations Center submits the investigatory packet to OPM for processing. At that time the investigation is considered œinitiated  and access can be provided. Contractors will be notified by OSL&E when the BI has been completed and adjudicated. B.3.3.3 The investigative history for contractor personnel working under this contract shall be maintained in the databases of either the Office of Personnel Management (OPM) or the Defense Industrial Security Clearance Organization (DISCO). Should the contractor use a vendor other than OPM or Defense Security Service (DSS) to conduct investigations, the investigative company shall be certified by OPM/DSS to conduct contractor investigations. B.3.4CONTRACTOR PRIVACY/SECURITY TRAINING: B.3.4.1 All Contractor employees shall complete the VA ™s on-line Security Awareness Training Course and the Privacy Awareness Training Course annually within the first 15 days of the contract period of performance. Contractors shall provide signed certifications of completion to the COTR during each year of the contract in addition to providing a signed Statement of Commitment and Understanding within the first 30 days of the period of performance for each employee. This requirement is in addition to any other training that may be required of the Contractor. (4). Contractor Equipment, Vehicles and Inspection: (a)The VA reserves the right to inspect Contractor ™s equipment and vehicles or require documentation of compliance with contract specifications, and State laws, rules, regulations and guidelines governing emergency medical transport vehicles (ambulances). VA inspections of Contractor facilities shall in no way constitute a warrant by the VA that the Contractor ™s vehicles and equipment are properly maintained. The VA reserves the right to restrict the Contractor ™s use of equipment and vehicles which are in need of repaired, unclean, unsafe, damaged on the interior or exterior body, or are not in compliance with contract requirements. The restriction of such equipment and vehicles shall not relieve the Contractor from performing in accordance with the strict intent and meaning of the contract without additional cost to the VA. (b) Ambulances shall meet all current applicable Federal, State and local specifications and regulations including, but not limited to, licensing, registration, and safety standards. Appropriate licensing by the State of Texas is required. (c) Vehicles shall be clean and maintained in good repair in accordance with manufacturer ™s instructions and specifications, at all times during the performance of this contract, and shall contain “ 1.An emergency safety kit. 2. Portable oxygen. 3.Fire suppression equipment. 4.Appropriate road emergency warning equipment. 5.Written emergency procedures. 6.Protective clothing required for the protection of Contractor employees where patient care is expected, such as “ i) gowns, ii) gloves, iii) masks, etc. (d)Basic Life Support (BLS) vehicle shall, in conjunction with the above, also provide on-board EMT, perform basis airway management, and provide Intra Venous (IV) and Keep-Vein-Open (KVO) without additives. (e) Advanced Cardiac Life Support (ACLS) vehicle shall, in conjunction with the above, also provide an on-board Paramedic, perform cardiac monitoring, provide ACLS drugs and procedures, perform advanced airway management and medication monitoring and administration. Advanced airway management means the ability to intubate a patient and provide appropriate respiratory care, which does not include tracheotomies, cricoidotomies or cricotracheotomies. (f) Wheelchair vans shall be equipped to safely load, carry and discharge VA beneficiaries. The Contractor is responsible for supplying all wheelchairs for wheelchair van transport, unless chairs are provided by the Medical facility or patient. Wheelchair vans shall contain at a minimum, the following: 1.Loading platform shall be integral to the vehicle and made of at thirteen (13) gauge steel. Platforms shall have raised edges, be counter balanced, self-adjusting to curbs and sidewalks, and self storing. When not in use platform shall be securely stored so as not to block the vision of the driver or inconvenience the patient. 2.Side and rear loading doors shall be operational from both inside and outside vehicle. 3.Vehicle shall have clamp cleats or belts to firmly anchor wheelchair and prevent movement in any direction. 4.Steps shall be treated with non-skid material. 5.Vehicle shall have safety belts for all occupants. 6.Vehicle shall have heating, air conditioning and adjustable temperature controls. 7.Vehicle shall contain four (4) emergency flares and warning lights, and one (1) five (5) pound ABC rated fire extinguisher with fire extinguisher tag showing record of inspections. 8.Vehicle shall contain a First Aid Kit with band aids, gauze, elastic bandages, sterile gauze pads, triangular bandages, cleansing wipes, tape, scissors, eye pads, and ammonia inhalants. All items shall be packed in sterile containers. 9.Vehicle shall have two (2) blankets. 10.Vehicle shall have on board supplies to provide infection control precaution procedures. 11.A two-way radio that shall be fully operational at all times during contract performance. (g) The Contractor that is providing ambulance services shall provide oxygen when it is requested by authorized Medical facility personnel or required by the patient ™s medical condition. Oxygen required for ambulance services shall be at the rate and price shown in section B.3, Schedule of Services and Price/Cost. (h) The Contractor shall not borrow medical equipment from the Medical facilities. The Contractor shall provide all medications, sheets, and blankets required for direct patient care while in transport. Contractor shall at no time and under any circumstances exchange supplies, equipment and/or medications with VA. The prices quoted in Schedule shall be inclusive of consumables used in transport. All vehicles used for critical patient care (ex. ALS, BLS), shall include, but not limited to, monitors, defibrillator with external pacemakers, intubation equipment, intravenous equipment, medications, patient compartment facilities, oxygen and suction system equipment including portable oxygen suction and accessories. 3. Escort: The VA reserves the right to have an escort, such as a relative, or care provider of beneficiary or VA staff accompany beneficiary when the VA determines that such an escort is in the best interest of the beneficiary. The VA will also be the sole judge in determining when an escort is required. There shall be no additional charge to the VA when escorts are authorized to travel with beneficiary. Contractor shall only be required to transport escort with patient and shall be not be required to return the escort back to point of origin. 4.Substitution of Beneficiary: The VA reserves the right to substitute the beneficiary requiring services at any time during the performance of this contract, to prevent delays, cancellations, or dry runs. There shall be no additional charge to the VA when such changes occur. 5.Number of Patients: (a)The Contractor shall transport one veteran beneficiary per trip, under ambulance services, unless specifically authorized by Medical facility personnel to transport more than one, and at no time shall the Contractor transport more than two patients. (b)When more than one beneficiary is transported on a trip in a single vehicle, reimbursement will be made at 70% of the base rate established in Part I ” Section B (Prices) for each (as noted under the Social Security Act (SSA) [42 U.S.C. 1302, 1395hh, 1395rr(b)(1)], Section 414.1). Incidents of trips with more than one (1) beneficiary are not expected to exceed eight (8) trips per contract year. The Contractors shall be entitled to a full base rate for each beneficiary traveling concurrently in a single vehicle once the maximum expected eight (8) annual trips have been exceeded. When travel is beyond the Medical facility ™s contract Mileage Threshold area of coverage, mileage may be claimed at 50% of the longest distance over which a beneficiary is transported in a single vehicle, for each, as noted in SSA reference. Incidents of trips with more than one (1) beneficiary, beyond the Mileage Thresholds, are not expected to exceed two (2) trips per contract year. The Contractor shall be entitled to separate mileage claims, at full rate for each, when more than one beneficiary is transported concurrently in a single vehicle once the maximum expected two (2) annual trips have been exceeded. 6.Hours of Performance and Response Time: (a)Hours of Performance: Contractor shall provide all ALS, Hired Car and Wheelchair Van transport services for veteran beneficiaries as requested during the following hours of operation: (i) Hours of Operation for Dallas “ Ambulance (ALS), Hired Car, and Wheelchair Van services are required between the hours of 6:01 AM and 6:00 PM, Monday through Friday, except on Federal Holidays. Ambulance (BLS) between the hours of 6:01 PM and 6:00 AM seven (7) days a week. (ii)Hours of Operation for FWOPC “ Ambulance (ALS), Hired Car, and Wheelchair Van services are required between the hours of 6:01 AM and 6:00 PM, Monday through Friday, except on Federal Holidays. (iii)Hours of Operation for Polk Street Annex “ Ambulance (ALS), Hired Car, and Wheelchair Van services are required between the hours of 8:00 AM and 4:30 PM, Monday through Friday, except on Federal Holidays. (b)Response Time: (i)The Contractor shall respond to all calls for ambulance transport service by being on-site at the designated point of pick-up within 30 minutes of notification. Should the time period for pick-up, stated herein, be physically impossible because of the location of veteran beneficiary the Contractor shall contact the VA travel coordinator within 45 minutes of the initial request and inform him/her of the anticipated time required for pick-up and delivery of the veteran beneficiary. If staging of the Contractor ™s vehicle onsite is necessary to ensure timely pick-up, this should be arranged by the Contractor in coordination with VA North Texas Health Care System. (ii)The Contractor shall provide and respond to all emergency calls in accordance with all Federal, State, and Local regulations governing dispatch of emergency medical (ambulance) vehicles and medical care of on-board patients, to include the use if sirens and other measures to ensure timely arrival at the designated VA Medical facility. (c)Response Time Compliance Rate: The Contractor shall maintain at a minimum, a 95% compliance rate on responses to pick-up or deliver during the contract year. Failure of the Contractor to perform in accordance with this compliance rating may constitute sufficient cause for termination (see FAR 52.212-4(m)). Failure to respond within one (1) hour of service call without notification to the travel coordinator shall result in reduction of 1/30th of the monthly invoice amount for each incident of non-compliance in excess of one (1) hour. This deduction reduces the monthly reimbursement by the estimated equivalent cost to purchase one (1) day of service from an alternative vendor. Failure to respond within one (1) hour time period on more than two (2) occasions during an invoice period shall result in default procedures set forth in FAR Part 49. (d)If the Contractor ™s place of business is located beyond the local telephone call zone of the VA (area codes 214, 817, 972, and 469), the Contractor shall install and provide to the VA, a toll free number for accepting the VA calls. It is estimated that 90% of all requests for service will be placed by telephone. Requests for service may also be in writing or oral from an authorized representative of the VA, and may be completed by telephone, facsimile, mail, e-mail, or in person (by giving a written or verbal request). (e)Electronic Transmissions: The Contractor shall, at all times during the contract period, have on his/her premises a functioning fax machine to communicate requirements with the VA (i.e., patient incidents, requests for service, special correspondence, etc.) The Contractor shall have a dispatcher available 24 hours days 7 days a week to receive request. The VA shall notify them of transportation by telephone request on 911 requests and/or scheduled transfer. The Bonham VA AOD will maintain a copy of all approved/authorized ambulance authorizations and medical officer that authorized the transportation. Changes will be communicated by telephone when applicable. 7.Waiting Time: (a)For unscheduled trips, waiting charges shall commence 15 minutes from the time the Contractor actually arrives at the designated pick-up and/or delivery points. For scheduled trips, waiting charges shall commence fifteen minutes from the scheduled pick-up time for from the time the Contractor actually arrives at the designated pick-up point, whichever is later. (b)If the designated pick-up and/or delivery points are at other than the VA Medical facilities and waiting beyond the required 15 minute grace period is anticipated, the Contractor shall notify the travel section. This call is only for the purpose of verifying the Contractor ™s time of arrival at pick-up and or delivery points and is not necessary if the Contractor does not anticipate a delay for which waiting charges will be claimed. The Contractor ™s failure to notify the travel section when waiting beyond the grace period shall result in non-payment for waiting time. 8.Authorized Medical Center Personnel: Prior to performance, the Contracting Officer shall provide the Contractor with a list of names or position titles and phone numbers of authorized Government personnel who may request services and receive calls from Contractor regarding contract performance. This list will be updated and submitted to the Contractor as changes occur. The Contractor shall ensure that requests for services are received from authorized medical center personnel. Services rendered in response to requests from other than authorized personnel shall be at the risk of the Contractor and any cost related thereto shall be borne by the Contractor. 9.Request for Services: (a)All requests will be placed against pertinent schedule items, on a unit price basis, and will be verified against invoices and Contractors request for service log submitted under paragraph 13. When ordering services (whether it be by telephone, facsimile, mail, e-mail and/or in person), the VA Medical facility will provide the Contractor with the following type of information: 1.Mode of transportation required; 2.Required date and time of arrival; 3.Name of beneficiary; 4.Pick-up and delivery point; 5.Type and number of additional medical care specialist required; 6.Type of equipment required; 7.Last four of the patient ™s Social Security Number; 8.Any other special instructions; and (b)The Contractor shall be required to transport non-ambulatory veterans on gurneys, litters and Geri chairs who are not in need of emergency transportation or medical attention. 10.Reimbursement for Mileage outside Contract Mileage Threshold Areas of Coverage: (a)Mileage Reimbursement ” Reimbursement for mileage outside the contract Mileage Thresholds of coverage, shall be at the mileage rate stated in the price schedule, and shall be based upon the miles from the pick-up point and the specified destination. Any mileage from the place of business and/or location of vehicle and the pick-up point, or form the delivery destination back to the place of business and/or location of next trip are not considered as a separate item of this reimbursement, but would be expected to be incorporated into the mileage rate as part of the administrative costs (or costs of doing business). Distances beyond the contract Mileage Threshold areas of coverage shall be calculated using current Rand McNally Standard Mileage Guide. Allowable charges for mileage outside the contract Mileage Threshold areas of coverage, shall not exceed ±5% of the current Rand McNally Standard Mileage Guide. (b)Should the VA make a determination that a previously scheduled trip may be cancelled, and a vehicle has already been dispatched to the designated pick-up point, VA may notify the Contractor to cancel the order. For orders that are cancelled while the Contractor is already in route to the designated pick-up, the Contractor shall be entitled to receive 50% of the base rate for the trip, as provided in Schedule of Supplies or Services and Prices/Costs. This charge shall not include any mileage charge. (c)Should the Contractor arrive at the destination before VA cancels the order, or if the Contractor is unable to perform a scheduled pick-up for reasons beyond the Contractors control, e.g., incorrect address, or patient absence, or patient refusal, then the Contractor shall receive 100% of base rate for a one-way trip as provided in Part I ” Section B (Prices), and applicable mileage if outside contract Mileage Threshold areas of coverage. (d)Other Charges ” it is understood that the prices listed in the accepted Schedule shall be inclusive of any and all ferry, bridge, tunnel or road toll charges. Contractor shall not bill the VA for any such separate charges. 11.Patient Rights: (a)The Contractor shall be courteous to VA beneficiaries ™ and shall not smoke while transporting patients. Patients may bring a reasonable amount of equipment, such as folding wheelchair, consumable medical supplies and personal suitcase. (b)The Contractor shall immediately notify the VA of any incidents involving injury to VA patients during transport. The Contractor shall promptly complete and submit to the COTR, an Incident Report ” Contract Ambulance Contractor with all information felt to be necessary for any full review. (c)The Contractor shall notify the COTR, in writing within 24 hours of any complaints made by the patients with regards to the ambulance service. The Contractor may provide recommendations for improved services along with the patient complaints for the VA ™s review. No recommendation shall be construed as being effective until and unless it is provided as a written modification to the contract from the Contracting Officer. (d)Upon delivery of a patient to the VA emergency room or patient ward Contractor shall log in the patient on a log sheet at the front desk. Contractor shall follow the same procedure when removing a patient from any VA unit for transport to other destinations. Contractor shall provide the following information on the log sheet: 1.Patient Name 2.Last Four of the patient ™s SSN 3.Time of delivery or pickup 4.Point of origin and destination 5.Name of Contractor personnel picking or delivering patient. 12.Infection Control Procedures: When ambulance services are required, the Contractor shall use universal precautionary measures for the prevention and control of the spread of infectious agents to all persons. 13.Required Reports and Documents: With seven (7) days after contract award the Contractor shall provide the following: (a)Contractor Personnel Roster “ In this document, Contractor shall identify the employee ” 1.Name 2.Position 3.Title, and 4.Work assignments area (b)Thereafter, any personnel changes shall be submitted within two (2) business days after the change(s) occur. (c)Certification of Minimum Liability Insurance Coverage: - A certificate of insurance as evidence of insurance coverage is required prior to providing any services to the medical facilities (ref. VAAR 852.237-71). Contractor shall provide the required proof of insurance along with his/her proposal. Thereafter, all changes shall be submitted within seven (7) days after changes occur. (d)Contractor shall provide a œRun Sheet , which will be included in the patient ™s permanent record. This Run Sheet shall include a record of the patient ™s vital signs, medications used, oxygen use, patient attitudes and any changes in the patient ™s condition. The list stated here shall not be construed to be as all inclusive; the Contractor shall provide all pertinent information. 14.Contractor ™s Quality Control Program (QCP): The Contractor shall establish and maintain a complete QCP to assure the requirements of this contract are provided as specified. An original and one (1) copy of this QCP will be forwarded to the CO along with the requested initial proposal. The CO will review the QCP and list any needed clarifications, and return to Contractor for response, if necessary. The Contractor ™s QCP shall include the following or have incorporated into the Contractor ™s QCP within three (3) months of beginning contract performance and identify at a minimum: (a)An inspection plan covering all services required by this contract. The inspection plan must specify the areas to be inspected on either a scheduled or unscheduled basis, how often inspections will be accomplished and documented, and the title of the individual(s) who will perform the inspections. (b)On-site records of all inspections conducted by the Contractor noting necessary corrective action taken. The Government reserves the right to request copies of any and/or each inspection. (c)Incorporation of either active or established internal policy and procedures for updating medical service protocols, that may affect performance of contract. (d)The methods for identifying and preventing deficiencies in the quality of service performed, before the level of performance becomes unacceptable, and organizational functions noting intermediate supervisory responsibilities and overall management responsibilities for ensuring total acceptable performance. (e)On-site records of all vehicle maintenance and repairs performed, on vehicles used in the performance of this contract. The methods of identifying and preventing vehicle breakdowns, and detailed procedure for alternative transportation of patients in the event of mechanical breakdowns, and detailed procedure for alternative transportation of patients in the event of mechanical breakdown of ambulance. SERVICE DISRUPTION: The Contractor shall immediately report to the COTR any and all vehicle breakdowns or other problems, which may cause service disruptions. When a breakdown occurs the Contractor shall provide a back-up vehicle to minimize any delay or inconvenience to the customer(s). When conditions, neither foreseeable and/or controllable by the Contractor occur, such as severe storms, flooding, hazardous road and travel conditions, time and distance requirements shall be considered secondary to safety. (f)On-site records identifying the character, physical capabilities, certifications and ongoing training of each employee performing services under this contract. (g)The methods of identifying and preventing radio communication breakdowns. A detailed procedure for alternative communications in the event of electronic and mechanical breakdown of vehicle two-way radios. (h)A log to account for all requests for service. The log shall indicate the date and time of service call, name of beneficiary requiring services, any required Medicare/Medicaid forms with Doctor ™s signatures, type of transportation requested, designated pick-up delivery points, actual time of arrival at pick-up and delivery points and actual waiting time at pick-up and delivery points, if waiting charges are claimed. (i)On-site records of any complaints or problems, with procedures taken to allow for corrections and/or elimination before effects caused interruption of performance of contract. 15.Miscellaneous: (a)At no time will the Contractor leave vehicles on VA premises unless a pick-up or delivery is in process. (b)Contractor employees shall conduct themselves in a businesslike manner at all times while on VA premises. (c)When transporting patients to or from Medical facilities, the driver, acting for the Contractor, will ensure that the patient ™s luggage, medical records, medications, and prosthetic devices are properly accounted for and delivered with the patient as required. 16.Mileage Threshold for Ambulance: Wheelchair Van Services “ The mileage threshold is a 75 mile radius form the designated pick-up point for each trip. Charges for travel within this mileage threshold shall be inclusive with the base rate. One-Way mileage for travel beyond the 75 mile threshold shall be claimed under the applicable mileage rate.
