- DOL Wage Determination No. 1995-0573 Representations and Certifications Experience Questionnaire Clauses Schedule of Items (Pricing) Statement of Work (SOW) This is a combined synopsis/solicitation for services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation document incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC) 2005-53. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The solicitation number for this procurement is AG-9AC7-S-12-0003 and is a Request for Quotation (RFQ). Submit written offers only; oral offers will not be accepted. Potential bidders are to provide a quote for the services outlined in the Statement of Work (SOW), which is included as an attachment to this Solicitation. Please submit pricing by means of the attached Schedule of Items (SOI). This acquisition is a 100% small business set-aside. The Northern American Industry Classification System (NAICS) code is 484210 and the size standard is $25.5 million. Only one contract will be awarded. A firm-fixed price purchase order will be issued. All firms or individuals responding must be currently registered with the Central Contractor Registration (CCR). Information concerning CCR registration requirements may be viewed via the Internet at http://www.ccr.gov or by calling the CCR Registration Center at 1-888-227-2423. DESCRIPTION: The Klamath National Forest is soliciting for quotes on the moving of office furniture, business machines, boxes and miscellaneous items from the existing Klamath National Forest Supervisor's Office at 1312 Fairlane Road in Yreka, California to the new office located at 1711 S. Main Street in Yreka, Siskiyou County, California in strict compliance with the terms, specifications, conditions, and provisions contained herein. The contractor shall furnish all necessary management, labor, equipment, supervision, transportation, supplies, and incidentals necessary to complete the work as specified. See SOW attachment. ESTIMATED START WORK DATE: This move will be accomplished in stages beginning on or about November 28/2011 and continuing through late December until all items are moved out of the 1312 Fairlane Road building. See tentative schedule outlined below in attachment - SOW. CONTRACT TIME: Approximately 24 calendar days. PRE-BID WALK THROUGH: A pre-bid walk through of the existing office is required of potential quoters. Site walk through is scheduled for Monday, October 24, 2011. Contact Debby Kottenbach at 530-841-4447 to schedule a visit of the site. EVALUATION FACTORS. The Government will award the contract to that responsible vendor whose quotation, conforming to the combined synopsis/solicitation, will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers: Price and Relevant Past Performance. The non-price evaluation factors, when combined, are approximately equal to price.
