- Attachment 3 Attachment 2 Attachment 1 Terms and Conditions This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation BPD-AFRG-12-CI-0003 is issued as a Request for Proposal (RFP). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. The NAICS code is 446110 and the size standard is $25.5. This is an unrestricted procurement. The Bureau of the Public Debt on behalf of the Armed Forces Retirement Home in Gulfport, MS (AFRG) is contracting to acquire Pharmacy Services in accordance with the attached Performance Work Statement. Period of Performance is for one base period, date of award through 09/30/2012 and two 12-month option years as follows: Date of Award - 09/30/2012 10/01/2012 - 09/30/2013 10/01/2013 - 09/30/2014 Work to be performed at the Contractor's facility. The provisions at 52.212-1, Instructions to Offerors - Commercial Items (JUN 2008), applies to this acquisition. Addenda to this provision are attached. The provisions at 52.212-2, Evaluation - Commercial Items (JAN 1999), applies to this acquisition. The following factors shall be used to evaluate offers: Technical Capabilities, Past Performance, Price. Addenda to this provision are attached.
