- STATEMENT OF WORK Joint Base Myer-Henderson Hall Ceremonial Organist 1. SCOPE OF WORK. The Contractor shall serve as Ceremonial Organist to support funerals and memorial ceremonies at Joint Base Myer-Henderson Hall on an as needed basis. Work shall be accomplished in accordance with the requirements set forth in paragraph 2 of this Statement of Work. a. Qualifications: The Contractor shall meet the following qualifications: (1) Have experience as ceremonial organist on a church/chapel setting. (2) Have experience performing funeral and memorial ceremonies. (3) Have excellent personal skills. b. Designation of Contracting Officer Representative: Chaplain (MAJ) David Santiago Cruz, 703-696-7671, or his authorized representative is designated as the Technical Representative of the Contracting Officer for the purpose of technical surveillance of performances and inspection of materials for work being performed under this contract. No inspector is authorized to change any provision of the specifications without written authorization of the Contracting Officer nor shall the presence or absence of an inspector relieve the Contractor from any requirements of the contract. c. Receiving Reports Responsibility: Contracting Officer Representative is responsible for preparing the receiving report and ensuring that the Contractor is paid monthly for the amount of services performed. The Contractor is solely responsible for turning in a monthly invoice of services to Contracting Officer Representative the following duty day after the last Sunday of each month. d. Termination for convenience of the Government: The Contracting Officer, with thirty days written notice, may terminate this contract, in whole or in part, when it is in the Government's interest. If this contract is terminated, the Government shall be liable only for payment under the payment provisions of this contract for services rendered before the effective date of termination. 2. CONTRACTOR PERSONNEL a. Competency of Contractor Personnel. All services under this contract are to be performed by an adult of at least 21 years of age. Applicants must be willing to undergo a background check. The Contractor shall be fully qualified to perform the required services without any supervision or direction by government personnel. b. Contractor Points of Contact. The Contractor shall provide the Contracting Officer with the names of a primary and alternate point of contact with telephone numbers, in case of emergency. 3. HOURS OF OPERATION. The Contractor shall provide the required products and services as indicated inthe delivery schedule of the contract. 4. GOVERNMENT-FURNISHED PROPERTY AND UTILITIES a. Office and equipment. The Government shall furnish chapel space at Joint Base Myer-Henderson Hall Chapels for funeral and memorial ceremonies/services and rehearsals. b. Utilities. The Government shall provide all utilities, such as electrical and water, to perform the services required by the contract, at the site of delivery of the services on Federal property. The Government will not make any modifications to utility outlets to accommodate Contractor's equipment. c. The Government shall not furnish the Contractor with government living quarters, office space or equipment. 5. CONTRACTOR-FURNISHED PROPERTY AND UTILITIES a. The Contractor shall furnish his/her own transportation to and from the work site(s). b. The Government shall not be liable for loss, damage, theft, or maintenance of personal equipment. 6. CONTRACTOR RESPONSIBILITIES a. The Contractor is solely responsible for all tax liabilities. No provision are implied or provided in this contract for any benefits such as medical care, retirement, or worker's compensation. This is a Non-Personal service contract. b. The Contractor shall regard personal information on all applicants, volunteers, and participants as confidential. As required by public laws and military regulations, the Contractor shall divulge information of illegal activities to the proper authorities. c. The Contractor shall present a neat, well-groomed appearance appropriate for chapel worship services during all times while he/she is performing services under this contract. 7. ADMINISTRATION. Administration for the purpose of this contract is the completion of reports,government forms, and other input that may be required for each contract product service. a. Document of Work/Reports. The Contractor shall maintain a log of the work performed under this contract, and shall provide a copy of the log to the COR on a monthly basis. b. The Contractor shall document worked performances on the Invoice or Request for Payment. c. Request for Supplies/Equipment. The Contractor shall provide the COR with the proper requests for the ordering of supplies and equipment to support the religious support of funerals and memorial ceremonies in accordance with the Consolidated Chaplain's Fund SOP and the Resource Manager's instructions (for appropriated fund supplies). (1) The Chaplain Resource Manager shall provide applicable forms, SOPs and instructions to the Contractor. d. Privacy Act. The Contractor shall be familiar with and ensure compliance with provisions of the Privacy Act, Freedom of Information Act, and limitations on government collection of personal and statistical data on nonaffiliated personnel. (1) Applicable government regulations concerning these acts shall be made available to the Contractor foreview by the Joint Base Chaplain. 8. JUSTIFICATION STATEMENT a. Description: Non-personal services contract for Ceremonial Organist in support of funeral and memorial ceremonies at Joint Base Myer-Henderson Hall. b. What is being procured: Ceremonial Organist Services for funeral and memorial services in support of Arlington National Cemetery funeral and memorial ceremonies/services. SCOPE OF WORK CEREMONIAL ORGANIST 1. Description of Agreement This contract is to be used to provide services of Ceremonial Organist as requested by the Contracting Officer or his authorized representative. Contractor shall perform as a Ceremonial Organist to support funerals and memorial ceremonies/services at Joint Base Myer-Henderson Hall on an as needed basis. 2. Invoices The individual invoices shall be submitted for each delivery. These invoices shall be accumulated and a consolidated payment will be made each month.
