Solicitation Number: VA260-12-Q-0073 Notice Type: Sources Sought Synopsis: AGENCY: Department of Veterans Affairs OFFICE ADDRESS: VISN 20 Contracting Office, 960 S. Broadway Avenue, Suite 460, Boise, ID, 83706 SOURCES SOUGHT NOTICE: VA260-12-Q-0073_1 CLASSIFICATION CODE: R Professional, administrative, and management support services NAICS CODE: 541690 This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation on part of the Department of Veterans Affairs. The purpose of this Sources Sought notice is to identify technically qualified businesses that can provide annual X-Ray Equipment Inspection Services for Spokane VA Medical Center, 4815 North Assembly Street, Spokane, WA, 99205. Spokane VA Medical Center anticipated X-Ray Equipment Inspection requirements are outlined below in the Draft Statement of Work. Responses to this notice should include the following: Name of company/provider, business size (large or small); identify if the company/provider falls into any of the following socioeconomic categories- Service-disabled veteran-owned small businesses; Veteran-owned small businesses; 8(a) small businesses; women-owned small businesses; HUBZone small businesses; or Small disadvantaged businesses; and the company/provider's size classification relative to the North American Industry Classification System (NAICS) code. ? Please include information on Medical Physics Services previously performed at any VA medical Centers and/or other government entities, to include contract numbers, dates of performance and points of contact. Include any other pertinent information which demonstrates the company/provider's ability to meet the above requirement. This is NOT a solicitation for proposals, proposal abstracts, or quotations; no formal solicitation documentation exists at this time. Responses to the information requested will assist the Government in determining the appropriate acquisition method, including if a set-aside is possible. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality: No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s)." DRAFT Statement of Work - Spokane VAMC Annual X-Ray Systems Inspection JCAHO Performance Evaluation - The medical x-ray equipment at thefacility will be evaluated in accordance with the following standards: "State of Washington's Office of radiation Protection, 246-225 WAC X-Rays in the Healing Arts. "National Council on Radiation Protection and Measurement Standards in NCRP Report No. 99 - QUALITY ASSURANCE FOR DIAGNOSTIC IMAGING. "American Association of Physicists in Medicine Protocol in AAPM Reports 1 - 57. "Joint Commission on Accreditation of Healthcare Facilities (JCAHO) Standards. The performance evaluations on the medical x-ray equipment includes but is not limited to the following tests: "Radiographic - Collimator congruence and accuracy, timer accuracy and reproducibility, exposure reproducibility, mA linearity on small and large focal spot mA stations, exposure measurements at various kVp stations, HVL measurement, AEC reproducibility, AEC minimum and maximum mAs settings. "Fluoroscopic - Tabletop entrance exposure measurements at all field sizes under normal and maximum operating conditions, CINE output measurements on all field sizes under normal and maximum operating conditions (if installed), resolution and contrast on all field sizes, spot film exposure measurements at various kVp settings, HVL measurements, ABS linearity and collimator accuracy in all fluoroscopic FOV and spot film modes. "CT Scanners - CT Dose Index measurements for various clinical techniques, HVL measurements, CT number accuracy and reproducibility, resolution and contrast determination, output uniformity about isocenter. "Portables - Collimator congruence and accuracy, timer accuracy and reproducibility, exposure reproducibility, mA linearity of mAs stations, exposure measurements at various kVp stations, HVL measurement. JCAHO Patient Dose Calculations - A table of patient dose calculations as required by JCAHO standards. These calculations are based on actual measurements performed during the JCAHO Performance Evaluation. This information is to be provided at no additional charge to the facility. Facility Evaluation - Review of required documentation - state regulations, registration on x-ray equipment, x-ray room posting. Review of film badge reports for the past year. Evaluation of dark rooms, processors, cassette and film bins. Evaluation of protective equipment. Review of department's radiation safety program. Policy and Procedures Review - Review the radiation safety standards in the department's Policy and Procedures Manual in accordance with JCAHO standards.
