Description
The Department of Veterans Affairs, VISN-20 VA Healthcare System is requesting information related to vendors capable of providing FIRE EXTINGUISHER MAINTENANCE SERVICES for the Walla Walla VA Medical Center (Walla Walla, WA and LaGrande, OR locations) in accordance with the statement of work below. This is not a request for bids, quotes, or proposals. This is a SOURCES SOUGHT announcement only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. The responses to this announcement will assist in determining the socio-economic set aside if feasible. The Veterans Administration is required by Public Law 109-461 to give first priority to SDVOSB and VOSB concerns if there should be sufficient firms available to compete under a set aside. If insufficient veteran owned and small business response is received to establish the basis for setting this action aside for other than large business participation then the action will be advertised as full and open competition, in which case, all interested parties responding shall be eligible to bid. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The vendors are requested to respond to the requirements for recycling as described in the statement of work below and submit related supporting information/documentation. Interested parties should submit with their response the following information no later than 9 January 2012 at 1100 MST and include POC information, business size, business SBA classification(s), DUNS number. STATEMENT OF WORK FIRE EXTINGUISHER INSPECTION AND SERVICE WALLA WALLA VA MEDICAL CENTER AS OF 20 DEC 2011 1.Contract Title: Fire extinguisher monthly inspections and annual servicing. 2.Background: As per NFPA 101 and VA guidebook, the Walla Walla VA Medical Center and LaGrande Community Based Outpatient Clinic (CBOC) fire extinguishers require monthly inspection and annual servicing. 3.Scope: Each fire extinguisher that is not in a hazardous area, such as a high-voltage vault, must be inspected each month and serviced once/year. Documentation will be kept by the contractor and at each extinguisher. 4.Specific Tasks: a.The Contractor will inspect each fire extinguisher monthly, no less than 26 days and no more than 34 days apart. This inspection will be documented at each extinguisher. An annual service of each extinguisher will occur no less than 340 days and no more than 390 days apart and be documented at the extinguisher. Extinguishers not meeting the inspection criteria will be serviced and placed back in their holder. b.Contractor to inspect each fire extinguisher not in a high-hazard area and service each of those extinguishers each year. Contractor will document the inspection. c.All inspections shall follow NFPA guidelines. d.There are 168 extinguishers to be inspected/serviced at the Walla Walla VAMC. e.There are 5 extinguishers to be inspected/services at the La Grande CBOC. 5.Performance Monitoring: Performance will be monitored by random inspections of the work by the Safety program. 6.Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI): The VA will furnish locations of each fire extinguisher included in the inspections. 7.Identification of Potential Conflict of Interest (COI): The contractor has the responsibility to ensure no potential Conflicts of Interest exist prior to performance of the work required for the contract. 8.Inspection and Acceptance Criteria: During random checks of fire extinguishers, performance will be monitored. 9.Place of Performance: Walla Walla Location: Department of Veterans Affairs Jonathan M.
