Solicitation Number: VA-241-12-Q-246 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA-241-12-Q-246 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-52, May 31, 2011. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv). The North American Industry Classification System (NAICS) code is 812332, and the size standard is $35.5 million. (v) This requirement consists of one(1) line item: CLIN (1) TO LAUNDER AND REPLACE MOPS AND DUSTERS ON A WEEKLY BASIS (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: STATEMENT OF WORK 1. Title of Project: Mops, Dusters, Towels and Hampers 2. Scope of Work: Contractor will provide 70 24 oz. forever mops per week; 50 microfiber high dusters cover per week; 1750 wet mops 18" microfiber per week; 150 wet tube mops medium size monthly; 2075 towels microfiber multi-purpose monthly; 300 dust mops 18" microfiber monthly; 200 dust mops 36" microfiber monthly; 4 storage hampers (at no cost; contractor will provide all handles/frames for dust and wet mops at no additional cost. All biohazard soil bags at no additional cost and all replacement for fair, wear and tear on all mops, dusters and towels at no additional cost. 3. Background: Based on previous services by Uni-First Corporation, 125 Etna Road, Lebanon, NH 03766. 4. Performance Period: JAN 20,2012 through September 30, 2012. 5. Type of Contract: Fixed-Price contract for services. 6. Place of Performance: VA Medical Center 215 North Main St. White River Jct.Vt. 05009 7. Performance Based Contract: This contract requires the application of an Earned Value Management System that is compliant with the American National Standards Institute /Electronics Industries Alliance (ANSI/EIA) Standard-748, Earned Value Management Systems.
