Nature and/or description of the item/service being procured and its intended use (include the anticipated cost):This purchase request is to obtain Medical Examination Services for Annual Asbestos physicals for Asbestos Workers in the Craft Shops located in Mammoth Hot Springs, Yellowstone National Park. These physicals consist of 1. Filling out an annual work sheet questionnaire, 2. A medical physical exam by a position for each employee, 3. Pulmonary Function test for each employee and a Chest X- ray (at the discretion of the Physician). 2. Sole Source Justification rationale (Check one and explain below): ___X__Only one responsible source. ______Urgent and compelling circumstances exist. Include critical delivery or performance dates, and when you became aware of the requirement. Provide a specific statement of the impact on Government operations, financial or otherwise, if the required delivery or performance dates are not met. Explain: There is a Medical Clinic Located at Mammoth Hot Springs within a few hundred yards of where all of these asbestos employees work. All of the employees that need these Asbestos Physicals are duty stationed in Mammoth Hot Springs. Each of these physicals takes approximately 1- hour to complete, so the Supervisor can make appointments for each of these 19 employees and these employees can basically work right up until the hour of their appointment. At the end of the appointment they can walk right back to work and continue with their work assignment; without any lost time for travel. Having these Physicals done anywhere else will require more time away from work and will have an additional cost for the government. Because having these physicals done anywhere else will require having to drive (more than 1- hour each way) in order to obtain these same services at any other medical facility. Having these physicals done right here at the Mammoth Clinic allows the government and the clinic to stagger these physicals when it is the most convenient for us and does not reduce our work productivity. IT allows us and the clinic to squeeze these appointments in of different days when we and the clinic are not bust or having to respond to an emergency. Another reason we are requiring that these services should be provided at the Mammoth Clinic is that if for some reason these appointments have to be changed or cannot be done at the appointed time, we don't have a bunch of employees standing around waiting on each other to finish their physicals. They can simply go right back to work and easily return to the clinic at the next opportunity. 3. Market Research. Please explain the results or why one was not performed. State whether any other offers were received or anyone expressed interest:We have had these physicals done at the Mammoth Clinic every year for more than 13 years.We reevaluate having then done at the Mammoth Clinic vs. elsewhere every year, but because of the additional time that the gov't would lose away from work and the additional costs for the transportation for all of these employees to another medical facility it is not feasible to look beyond Mammoth Hot Springs.These services are available at the clinic located at Mammoth Hot Springs, all the employees are duty stationed at Mammoth Hot Springs, all of these employees can continue to work right up until it is time for their appointment and immediately after their appointment can go right back to work without any lost time driving more than 1-hour each way to obtain the same services that are available a few buildings away from where they work.4. If this is a determination for sole source, the Contracting Officer certifies: a.Only one source is available, or urgent and compelling circumstances exist. b.The anticipated price will be determined fair and reasonable using the price analysis techniques described in FAR Part 13.106-3(a).
