- Sources Sought: The VISN 6 Network Contracting Office intends to solicit vendors to provide WalkAway 40 Plus System Cost Per Test Panel Agreement as listed above in accordance with Public Law 104-262 and 38 USC 8153 for the Department of Veterans Affairs Medical Center, Durham, North Carolina. Contractor and all sub-contractors must be licensed and capable of providing antibiotic susceptibility testing to identify organisms in patient specimens. Offerors must provide copies of licenses and certifications along with the offer. The delivery order shall specify the item and quantity of the items delivered and the time of delivery. Proposals that do not contain above requested licenses and certifications will be considered non-responsive and will not be considered for award. Additionally, only offerors will be consider if you're able to meet the time constraints specified in the Statement of Works below: STATEMENT OF WORK (SOW) PURPOSE: The purpose of this contract is the contractor shall provide a cost-per-test agreement and automated support for antibiotic susceptibility testing in the Durham VA Medical Center Microbiology Lab to identified organisms patient specimens. The cost-per-test agreement shall include inoculators, controls, supplies, and preventive maintenance service for a WalkAway-40 plus Analyzer. The analyzer shall be maintained by contractor in accordance with manufacturer's standards. 1. B1018-340 The WalkAway-40 Analyzer brand name or equal possessing the salient characteristics that offer: a.Simultaneous automation of conventional overnight, Synergies plus ®, Rapid, and specialty panel tests b.Automatic incubation, test interpretation, and reagent control that will free staff for other tasks c.Up to 40 panel capacity models d.All equipment parts and accessories needed to operate the analyzer, to include an UPS 2. B1018-516 LabPro Alert Module 3. Services to be provided: a.1 Year Business Hours Service and Preventive Maintenance Agreement - The Analyzer preventive maintenance to be serviced to function in accordance with manufacturer's standards b.Timely Service and support for the equipment, within one hour call back service. c.Replacement parts as needed. d.Troubleshooting of equipment 24/7 e.Training and technical support for the system. f.Updates and upgrades as available 3. B1013-4 18 Units of Inoculators 4. B1026-10D 70 Units of Inoculum 5. B1017-405 210 Units of Negative MIC 32 6. Training: a. Initial training of two primary operators b. One additional training per contract renewal year
