- Appendix D Amendment - Quality Assurance Worksheet Appendix A Amendment - Requirement Sechdule Amendment 2 AG-82MK-S-12-0003 is for Janitorial Services for the Collbran Work Center on the Grand Mesa, Uncompahgre and Gunnison National Forests. The work center is located at 209 High Street in Collbran, Colorado. This Janitorial Services contract shall include all management, tools, equipment and labor necessary to ensure that janitorial services are performed in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. A pre-proposal site visit is scheduled for January 19th, 2012 at 2:00 p.m. Details of the meeting will be included in the Solicitation. This pre-proposal is NOT mandatory. All participants must RSVP by January 18th, 2012 at 1:00 p.m. by contacting Ann (Peggy) Dobie at 970-642-4415. This is a total small business set aside. The anticipated date for award on this solicitation is February 29, 2012. The due date is identified in the solicitation. The solicitation is available for free download at www.fbo.gov. All quesitons pertaining to this solicitation must be submitted in writing to Peggy Dobie at pdobie@fs.fed.us or via facimile to 970-642-4425. All Offerors shall be registered in the Central Contractor's Registry (CCR) in accordance to FAR 52.204-7 to be considered responsive to this solicitation. All Offerors must be registered in ORCA in accordance to FAR 52.204-8 to be considered responsive to this solicitation. All Offerors must respond to all evaluation criteria in Section M of the solicitation to be considered responsive to this solicitation. Offerors who fail to meet these three criteria will not be considered in the Technical Evaluation of Offers.
