The National Cemetery Administration is soliciting offers under Solicitation No. VA-786-12-R-0075 for the following service: The purpose of this contract is to supply blank granite niche covers, 6-hole, for Lanai & Hoolehua Veterans Cemeteries at Hilo, Hawaii. These items are to be delivered, F.O.B. Destination, within consignee's premises to: Isemoto Construction, Inc. 484 Kanoelehua Avenue Hilo, Hawaii 96720 Potential offerors will submit, at contractor expense, one granite niche cover as a sample representative of the work to be done, the sample should be in accordance with the specifications listed in the solicitation. This sample should be received and approved prior to the closing date of the solicitation. Evaluation criteria for this award are 1) Price, 2) Past Performance, and 3) Technical Qualifications. Price and Past Performance are equally important. This acquisition is 100% set aside for Small Business (SB). The NAICS Code is 327991. Size standard is 500 employees. Electronic solicitations will be available for download from http://www.fbo.gov on or about February 21, 2012. Responses to the solicitation are due on or about 2:00 P.M. on March 8, 2012. Any amendments to the solicitation will also be available at www.fbo.gov. Offerors are advised they are responsible for obtaining any amendments. Contractors shall follow submission instructions for proposal as written in Section E.3. Any missing requirements may cause the Contracting Officer to consider the proposal to be non-responsive and exclude the proposal for consideration. ****NOTE - All potential offerors who have submitted and received approval notification for a sample of same type within the last 12 months may submit a copy of that approved review to MPS to avoid submitting another sample.*****
