Description
Solicitation NIHLM2012302-UHP - Specifications This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information contained in this notice. This announcement constitutes the formal Request for Quotation NIHLM2012302-UHP and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through February 2012 Federal Acquisition Regulation revision, which includes the consolidation of all Federal Acquisition Circulars 2005-55. This acquisition is a 100% small business set-aside and the NAICS code is 322211. It is the intent of the National Library of Medicine (NLM) to negotiate with a contractor to provide library supplies for rare books and early manuscripts for the History of Medicine Division (HMD) at NLM. Samples of every item on which a quote is being placed should be submitted for test purposes. Vendor must maintain an inventory of products and be able to provide order fulfillment for individual orders. These orders must be filled within 3-5 business days. Individual orders will consist of at least 100 items. This inventory will be the sole responsibility of the vendor. Vendor will be responsible for maintaining order records and invoice once a month for payment for product, handling and shipping. Addition cost for storage and shipping must be priced separately. Any failed supplies or supplies not meeting stated requirements will be returned for replacement at a contractor's cost. It is anticipated that MULTIPLE IDIQ AWARDS will be made from this solicitation. Individual orders will be firm fixed-price. This requirement is for one base year with options for four (4) successive one year periods. The period of performance is on or about March 12, 2012. The offeror shall include all information which documents and/or supports the qualification criteria in one clearly marked section of its proposal. ** The specifications of this requirement is attached file to this solicitation ** All proposals received by March 2, 2012, 4:30 P.M. (Eastern Standard Time) will be considered by NLM. Responses must be in writing, and must include pricing information and meet the criteria requirement. Offerors may propose all or any combination of the supplies as stated in the attached file, but proposal must clearly specify the supplies being offered. All responses from responsible sources will be considered. Inquiries regarding this procurement may be made to the National Library of Medicine, Office of Acquisitions, 6707 Democracy Boulevard, Suite 105, Bethesda, Maryland 20894-5488 (20817 for courier services), and Attention: Uyen H. Phuong, Contracting Officer, on (301) 496-6127. Please submit ONE (1) signed original and THREE (3) copies of the proposals on or before 4:30 PM EST, March 2, 2012 to: Uyen H. Offerors are advised to provide with their offer a completed copy of the following provisions: FAR 52.212-3, Offeror Representations and Certifications Commercial Items (May 2011)-With DUNS Number Addendum [52.204-6 (April 2008)].
