- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W81K00-12-R-M004 and is issued as a Request for Proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55 and Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20120130. This solicitation is for Test Cards and Equipment for the San Antonio Military Medical Center (SAMMC), Fort Sam Houston, Texas. Specifically, the requirement is for the BioMerieux Vitek 2 Antimicrobial Susceptibility and Identification Testing System. The base of period performance is 1 April 2012 through 30 September 2012, plus two twelve-month option periods (1 October 2012 - 30 September 2013 and 1 October - 30 September 2014) and one six-month option period (1 October 2014 - 31 March 2015). The associated North American Industrial Classification System (NAICS) code for this procurement is 325413, In-Vitro Diagnostic Substance Manufacturing. The size standard for NAICS 325413 is 500 employees. This procurement is being conducted on an unrestricted basis. All eligible businesses may submit an offer, which will be considered. Offers are due by 19 March 2012, at 12:00 PM, Central Standard Time. The point of contact is Ms. Michelle Bartkowski at (210) 221-3249 or Michelle.Bartkowski@us.army.mil. Offers shall be submitted via e-mail. Questions shall be submitted via e-mail not later than 5 March 2012. No questions will be entertained after this date. This is a "brand name or equal" solicitation. As indicated in the Schedule, the brand name description of the products are BioMerieux. The successful offeror shall provide all necessary equipment, installation, any work necessary to maintain the equipment, training, and 24/7 support, as required to conduct the necessary testing (as demonstrated by the items on the Schedule below) at no cost to the Government. It is the intent of the Government to establish a "Cost per Test" contract as a result of this solicitation.
