SOURCES SOUGHT SYNOPSIS REQUEST FOR INFORMATION ONLY THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. THE VA IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. In accordance with FAR 15.201(e), under Exchanges with Industry before Receipt of Proposals, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. The Department of Veterans Affairs (VA) is seeking potential sources capable of providing Ambulance Services as well as twenty-four hour service, seven days a week, for the beneficiaries of the VA Butler Healthcare System. Synopsis: 1. VA Butler Healthcare is seeking sources to provide Ambulance Service for basic life support transport, advanced life support transport, and wheel chair van, including all necessary personnel, vehicles, equipment, materials, and supplies, to the Department of Veterans Affairs, in accordance with all contract terms, conditions, provisions and requirements. The contract will be for base year plus one (1) option year to renew. The contractor will be required to furnish services within 30 minutes after receiving any request, the VA reserves the right to obtain the service from another source and to charge the contractor with any excess costs. However, the contractor will not be charged with any excess costs, if the VA has already placed requests for four (4) ambulances within a four (4) hour period and five (5) wheelchair vans within a four (4) hour period. If all wheelchair vans are in use, and a wheelchair van is requested, the contractor shall provide an ambulance at the van rate. The VA will be the sole judge in determining when to order service from another source-especially in instances of cardiac or emergency cases. Furnish ambulance and wheelchair van service to eligible VA Beneficiaries of the Department of Veterans Affairs, 325 New Castle Road, Butler, PA 16001, 24 hours a day, seven days a week. The intent of this contract is to furnish ambulance services, for basic life support transport, advanced life support, and wheelchair van services, including all necessary personnel, vehicles, equipment, materials, and supplies. Medical center working hours are Monday through Friday, 8:00 a.m. to 4:30 p.m., weekends and holidays; however, the contractor is required to provide service 24 hours day, 7 days a week, including normal duty hours and off duty hours, weekends and holidays. The contractor will be required to provide basic life support technicians and or drivers, and other contractor personnel. Technicians and drivers must be capable to administer oxygen and have successfully completed the Standard Advance First Aid Courses of the American Red Cross or U.S. Bureau of Mines, or equivalent. The contractor must ensure that every trip must include one Emergency Technician and Paramedic for every transport. 2. Personal must have completed training in accordance with the standards published by the Department of Health and Human Services with a minimum curriculum 81 hours or "equivalent" including an in hospital training period. 3. The place of performance includes the following counties in Pennsylvania: Allegheny and Butler County and all surrounding counties. VA Medical Center, University Drive C, Pittsburgh, OPA 15240, VA Medical Center, 7180 Highland Drive, Pittsburgh, PA 15206, VA Medical Center (Aspinwall), 1010 Delafield Road, Pittsburgh, PA 15240 and VA Butler Medical Center, 325New Castle Road, Butler, PA 16001. 4. The NAICS Code associated with this request for information is: 621910 5. Responses to this RFI should be emailed to Cynthia.lind3@va.gov not later than February 28, 2012 and should include the following information: a. Name and address of company; b. Contract will be in compliance with all VA security policies and applicable confidentiality statues, including 38U.S.C 5701, 38 U.S.C 5705, 38 U.S.C. 7332, 5 U.S.C. 552a (Privacy Act), as well as 45 C.F.R. Parts 160, 162, and 164 (Health Insurance Portability and accountability Act). Contractor must adhere to the provisions of Public Law 104-191, Health Insurance portability and Accountability Act (HIPPA) of 1996 and the National Standards to Protect the Privacy and Security of Protected Health Information (HHS) has promulgated rules governing the security, use and disclosure of protected health information by covered entitles, including the Department of Veteran Affairs (VA). c. Business size; d. GSA/FSS Schedule number. (if available) e. Identification of any socioeconomic categories (SDVOSB, VOSB, 8a, Hub, WOSB, SB) the company falls under. f. Any other pertinent information. g. A letter or memorandum indicating a positive intention to provide a proposal if requirement is solicited. The letter should be signed on behalf of your company by a senior procurement official, senior management, owner, or partner (an individual with authority to bind the firm). PLEASE LIMIT RESPONSE TO 10 PAGES 6. Response date: February 28, 2012. Responses must be received no later than 4:00 p.m. on Tuesday, February 28, 2012. Responses received after, February 28, 2012 may be included with our research but any future consideration for this procurement is not guaranteed if the response is received after the deadline. 7. Veterans First Contracting Program In accordance with IL 049-07-08 "Veterans First Program" the VA, will place SDVOSBs and VOSBs first and second, respectively, in satisfying VA's acquisition requirements when possible. Certain conditions must be met. All VOSBs, including SDVOSBs, must register in the VetBiz.gov Vendor Information Pages (VIP) database, available at www.VetBiz.gov, and CVE certified to be eligible for award of a contract. 8.
