- ******AMENDMENT TO PREVIOUS SOURCES SOUGHT NOTICE****** The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is seeking to obtain information on potential sources to provide architect and engineering design services for "ICU/Medical/Surgical Unit, Project #595-301". Sources shall have capabilities to provide architect and engineering services to design an interstitial floor and a 27-bed ICU/Medical/Surgical unit on top of the current Surgery Building. The project will be in the range of 25,000 to 30,000 square feet and will have complicated phasing. Project services will include site investigation, completion of bid documents and construction period services. This is not a request for competitive quotes. This is a notice to obtain possible sources that have the capacity & capability of providing the architect and engineering design services as described in this notice. Information regarding your firm's capacity, capability, and past experience to provide such services are to be submitted electronically on or before Thursday, February 23, 2012 prior to 4:00 pm (EST) addressed to Laura Harding, Contract Specialist: laura.harding@va.gov. Provide documentation showing performance of prior design completion on projects of similar size, scope and magnitude as to that described above that were constructed at a cost of greater than $7 million. Please submit your firm's experience in an email to laura.harding@va.gov as follows: Subject Line: Sources Sought Response, Project #595-301 - ICU/Medical/Surgical Unit. Please submit relevant information on your firm's letterhead as an email attachment. This should be submitted in.pdf format, not to exceed two (2) pages in length. Please list your firm's socioeconomic status, if applicable (SDVOSB, VOSB, WOSB, HUBzone, etc.). The North American Industry Classification System Code (NAICS) is 541310 and the small business size standard is $4.5 million. The request for SF330 packages will be available approximately on or about April 1, 2012 and should be downloaded from this website at that time.
