This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-12-Q-0123 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-55. Only quotes received by email from offerors are acceptable. Email your quotations to lisa.cross@va.gov. No telephone or FAX quotations will be accepted. The North American Industry Classification System (NAICS) number is 452910 and the business size standard is $27.0 million. All quotations must state your business size. Note: The proposed contract listed here is Set-aside 100% for Small Business. Line Item 1 (of 1) - Contractor shall provide 726 each (EA) $50 Meijer brand gift cards to the VA Ann Arbor Healthcare System, Medical Center, 2215 Fuller Road, Ann Arbor, MI 48105. FOB Destination, Terms NET 15. Gift cards must be received by VA Ann Arbor Healthcare System within 15 days after receipt of order (ARO). These gift cards are provided to participants in research studies, so these gift cards shall be plastic, rectangular cards that can be given to patients. (Electronic cards shall not be accepted). All questions regarding this RFQ shall be sent to lisa.cross@va.gov no later than 3:30pm on Monday, Mar. 5, 2012. Offerers must comply with all instructions contained in provision 52.212-1, Instructions to Offerers-Commercial Items, applies to this acquisition. The following additional provisions and clauses apply: 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 52.204.10, 52.219.6, 52.219-28, 52.222-3, 52.222-19, 52.222-36, 52.223-18, 52.225-1, 52.225-13 and 52.232.34. Attention: You must be registered in CCR to be considered for award. The following additional clauses apply: FAR 52.207-4, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide its DUNS or, if applicable, it's DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the CCR database. (C) Lack of registration in the CCR database will make an offeror ineligible for award. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
