- REQUEST FOR INFORMATION, SOURCES SOUGHT THIS IS A REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT ONLY AND YOUR RESPONSE IS NOT AN OFFER AND DOES NOT COMMIT THE GOVERNMENT TO PAY ANY COSTS INCURRED IN PREPARATION OF ANY SUBMISSION TO THIS MARKET RESEARCH, OR TO CONTRACT FOR SUPPLIES/SERVICES. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to determine if there are a sufficient number of qualified Small Business firms to set aside a Indefinite Delivery Indefinite Quantity (IDIQ) contract. The Government will use responses to this notice to make to appropriate acquisition decision. The selected firm shall provide all labor, materials, transportation, equipment and supervision, in compliance with federal, state and local regulations, to provide Custom Sterile Surgical Packs (custom procedure trays) for the following VA medical facilities: Denver VAMC, Colorado, Grand Junction VAMC Colorado, Salt Lake city VAMC Utah and Cheyenne VAMC Wyoming. The nature of this contract typically requires numerous changes to, as well as discontinuance and replacement of existing packs. Therefore, offerors submitting proposals shall be primarily engaged in the business of direct assembly of custom surgical packs for medical facilities. Offerors shall be able to demonstrate to the government that their firm has historical track record and industry expertise in the custom surgical pack assembly business. Furthermore, the offeror shall demonstrate that, as a direct assembler of custom surgical packs, the offeror can meet the quality assurance, sterilization and environmental control standards specified in the solicitation. The offeror shall be required to submit non-sterile bid samples as part of their proposal. All interested firms who can meet the requirements stated above should respond, in writing, with a capability statement describing their interest, capabilities, geographic location and indicating the business category in which the firm belongs. The capability statement should be sent to the Contract Specialist Kim Cooper no later than 3:00 p.m. (Mountain Time), 20 March 2012. Solicitation VA259-12-R-0078 OR CUSTOM SURGICAL PACKS has been assigned and should be referenced on all correspondence regarding this announcement. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 339113 is applicable to this acquisition; the size standard for a firm to be considered under this NAICS Code is a business size of 500 employees. The firms that are interested in submitting a response to this announcement has to be. In accordance with FAR 19.102 (f) (1)&(2). Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database prior to award of a contract. Below are questions to be answered when submitting your capabilities: 1. Will you have a local company representative, if not, how will local issues be addressed that may come up where a local representative may be invaluable? 2. Are you capable of substituting other vendor's items in the packs? (For example purposes: a certain 3M suction tip is needed for cardiac cases, it cannot have a Medline suction substituted just because. It has to be an exact match in every way if there is any chance of approving that kind of substitution.) 3. Will you guarantee and be able to meet delivery dates? (Example: 3 day ARO, if not what is your standard delivery timeframe?)
