- THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes - it does not constitute a Request for Proposal (RFP) or a promise to issue an RFP in the future. This request for information does not commit the Government to contract for any supply or service whatsoever. Further, the DoD Domestic Dependent Elementary and Secondary Schools (DDESS) is not at this time seeking proposals and will not accept unsolicited proposals. Respondees are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party's expense. Not responding to this RFI does not preclude participation in any future RFP, if any is issued. If a solicitation is released, it will be synopsized on the Federal Business Opportunities (FedBizOpps) or GSA E-Buy website. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. Scope: This Sources Sought is for nursing services (RN or LPN) for School Year (SY) 2012-2013 at West Point Elementary School. General dates are mid-August through 31 October 2012 with a break from 1 November 2012 to 31 March 2013. Services will resume 1 April 2013 until 5 June 2013. This requirement will be for 5 days per week, 6 hours per day from 8:00AM until 2:00PM for the continuous care of a Kindergarten student in the classroom. Particular Nursing training includes but not limited to: • administration of all medications • suctioning of a trach continuously • constant monitoring of compromised breathing (student must be given oxygen immediately if her blood oxygen levels go below a specific level - an oxygen tank is attached to her wheelchair at all times) - oxygen levels are checked constantly via a pulse ox finger tab • feeding via a G-tube • strategic positioning to/from wheelchair onto a mattress, wedge, etc. placed on a table in the Kindergarten classroom so as not to affect breathing or cause bone breakage (an associated dx is a bone disease resulting in brittle bones) • identification of signals when student is in distress and cannot speak due to compromised breathing Diagnosis of Student: Spinal Muscular Atrophy (SMA) is a progressive disease in which the spinal column is disintegrating over time. This disease impacts breathing, mobility, and all daily living skills. Breathing is a particular concern when the student becomes ill (she is immuno-suppressed and is more susceptible to illness when exposed to germs). The disease has progressed thus far to a point where the student can no longer be in an upright position for more than a short duration of 20 minutes. She comes to school in a reclined specially-designed wheelchair. Continuity of care is of the utmost importance. Communication with the school and the home nursing staff must be coordinated prior to start of services. Requested Information: 1. Interested vendors 2. Comments on these draft requirements 3. Number of nursing staff available to meet this requirement Administrative: All interested small business sources must send a notice of interest via E-Buy, prior to this E-Buy RFI notification closing on 4/23/2012. By submitting an E-Buy response acknowledging an interest in this acquisition, you must have the required GSA Schedule 621 I, with SIN 621-025 or 621-038 contract. Interested vendors will be added to the sources list. Background: The Department of Defense Education Activity (DoDEA) is a Department of Defense field activity operating under the direction, authority, and control of the Deputy Undersecretary of Defense. DoDEA provides education to eligible Department of Defense (DoD) military and civilian dependents from preschool through grade 12. The focus of the DoDEA organization is to provide quality education to all eligible students. The DDESS system serves an estimated 36,400 students in 70 schools located in seven states, Guam, Cuba, and the Commonwealth of Puerto Rico.
