Description
This is a combined solicitation/synopsis for following: Technetium Generators 7.5 and 3.0 Curie, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-12-T-0740 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 325412; Size: 750. All interested bidders shall submit quotations electronically by email to joseph.bancod@med.navy.mil or by facsimile at 619-532-5596, attention Joseph Bancod. E-mail submissions are limited to 2MB. The submitter should confirm receipt of facsimile and email submissions. Quotations are due on or before 26 April 2012, at 08:00 AM Pacific Standard Time to be considered responsive. Please see schedule below: May 2012: 7th, 14th, 21st, 29th. Jun 2012: 4th, 11th, 18th, 25th. Jul 2012: 2nd, 9th, 16th, 23rd, 30th. Aug 2012: 6th, 13th, 20th, 27th. Sep 2012: 4th, 10th, 17th, 24th. CLIN 0002 Technetium Generator,3.0 Curie QTY 22 EA $______ Delivery one(1) Generator on each date below: May 2012: 2nd, 9th, 16th, 23rd,30th. Jun 2012: 6th, 13th, 20th, 27th. Jul 2012: 5th, 11th,18th,25th. Aug 2012: 1st, 8th,15th, 22nd, 29th. Sep 2012: 5th, 12th, 19, 26th. EXTENDED DESCRIPTION: Each generator shipment shall include twelve (12) vials of each of the following items at NO charge to the Government: 1. Evacuation vials. 2. 5.8ml Saline vials. 3. 20ml Saline vials. All deliveries shall be made directly to the Nuclear Medicine Clinic at the Naval Medical Center San Diego, Building 1, 2nd Floor. All radiopharmaceutical products must be free from all defects. All supplies/Tech Generators shall be delivered with calibration date and time, and an expiration date and time, as a lot number, package insert, and packing list. Supplies/Tech Generators shall be received by one of the nuclear medical staff. If items are not needed on any particular day, the Government has the right to cancel or make any adjustments as necessary to supply orders/Tech Generators within a 24 hour time period. Contractor must be registered to the Central Contractor Registration (CCR) prior to award. All responsible sources may submit a quotation which shall be considered by the agency Performance Work Statement 1.0 SCOPE: 1.1 The purpose of this contract is to provide nuclear medicine isotopes for the sole purpose of developing radiopharmaceuticals within the Nuclear Medicine Clinic Radio pharmacy at Naval Medical Center San Diego. The radioactive pharmaceuticals are used for diagnosis and treatment of patients with medical conditions. These supplies will also help to provide quality patient care and help doctors with the process of surgeries and treatment of patients. Generators for Medical Imaging must be FDA approved and fall under the Nuclear Regulatory Commission (NRC) standards. 2.0 OBJECTIVES: 2.1 Generators for Medical Imaging shall be delivered in a timely manner. All expired generators shall be delivered back to the contractor Medical Imaging via approval and inspection of NMCSD Radiation Safety Officer. 2.2 All generator products shall be free of defects. 2.3 All supplies shall be delivered with a calibration date and time and an expiration date and time, lot number, and package insert and a packing list. 3.0 DELIVERY REQUIREMENTS 3.1 Delivery times at NMCSD Nuclear Medicine Clinic: 3.0 Curie Delivery One (1) Generator on each date every Wednesday of the month. 3.2 Each shipment of Technetium Generator will include twelve (12) vials of each of the following at NO charge to the Government: 1.Evacuation Vials 2.5.8ml Saline Vials 3.20ml Saline Vials 3.3 All supplies shall be delivered with a calibration date and time and an expiration date and time, lot number, and package insert and a packing list. 3.4 Delivery Schedule for the next five months is as follows: 3.4.1 7.5 Curie Generator. Deliver one (1) Generator on each date every Monday of the Month.
