Description
The project listed below has been amended to correct the submittal requirement date for proposals. Interested firms must submit two copies each of current SF-330 no later than 4:30 PM local time on May 2, 2012. In addition the correct issuance date for the solicitation is April 19, 2012. The Southeast Louisiana Veterans Health Care System is seeking Architect-Engineering (A/E) design services for Project VA-256-12-R-0456 Architectural and Engineering Services necessary to prepare complete contract documents consisting of specifications, drawings, and any other documents for bidding purposes and A/E construction period services to upgrade the Urgent Care Center, New Orleans, LA. The Architect/Engineer (A/E) firm shall provide all design/engineering services, investigation, design surveys, verification of as-built conditions, construction drawings, cost estimates, phasing and estimated construction time schedule as required for a fully developed set of construction documents for the renovation of an 8,000 square foot clinic to include utility systems and related infrastructure, refurbishment of interior space and finishes, and full operation for patient services and support functions. Work includes, but is not limited to: refurbishment of all wall, floor, and ceiling finishes, repair and painting of exterior finish, general replacement of HVAC, roof, Mechanical, Electrical, Telecommunications, and Fire Protection Systems modifications as necessary. A/E shall specifically include in the contract documents the requirement for all necessary inspections, re-inspections, certifications, and functional test for a fully operational space than meets all local building codes, NFPA, Joint Commission, and VA requirements. A/E selection based on recent specialized experience, professional capacity to accomplish work, professional qualifications of staff and consultants assigned to the project, proposed management plan, cost control, estimating effectiveness, schedule control and location of A/E firm and consultant. NAICS 541310 applies - size standard is $4.5 million. This acquisition is 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB). All offerors must be listed as "verified" in the VIP database, available in VETBIZ at http://www.vip.vetbiz.gov and must also be registered at CCR http://www.ccr.gov under NAICS 541310 to submit offers and be eligible for award. The A/E is also required to submit Annual Representations and Certifications via the Online Representations and Certification Application (ORCA) at https://orca.bpn.gov. Registration in ORCA requires and active Central Contractor Registration (CCR) record. Interested firms should submit two copies each of current SF-330 to Attn: Cynthia White, Contracting Specialist (90C), VA Health Care System, P.O. Box 61011, New Orleans, LA 70161 no later than 4:30 pm local time on MAY 2, 2012. The A/E selection criteria shall include your qualifications and past performance data. The A/E selection based on the following criteria: recent specialized experience, professional capacity to accomplish work, professional qualifications of staff and consultants assigned to the project, proposed management plan, cost control, estimating effectiveness, schedule control and location of A/E firm and consultant (A/E firms must be within a 100 mile radius driving distance from 1601 Perdido Street, New Orleans, Louisiana). The estimated construction cost range is $500,000 - $1,000,000. THIS IS NOT A REQUEST FOR PROPOSAL (RFP). Solicitation No. VA-256-12-R-00456 will be issued on or about April 19, 2012 and the approximate proposal due date is MAY 2, 2012. The completed SF-30 will be evaluated by the VA Medical Center Engineer Evaluation Board. NOTE: This project is subject to the AVAILABILITY OF FUNDS.
