Description
GENERAL INFORMATION Title of Project:Bathroom remodel for Independent Living Scope of Work:The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The overall purpose of this project is to make certain modifications to the veteran s residence, located at 4333 Juniata Street, St. Louis, Missouri, to allow for greater ease in movement consistent with meeting the veteran s independent living needs. Background:The work authorized pursuant to this statement of work represents a portion of the home modification work being performed by Contractor at this veteran s residence. Only those remodeling tasks identified herein are authorized to be performed under this contract. Veteran s Bathroom Build accommodations to an existing approximately 6 feet wide by 15 feet long bathroom that will be handicap accessible. Install ADA Toilet using existing lines, drains, and stacks, Install walk in shower unit with single lever faucet, shower head, and hand held shower unit. Equipped with shower rod, weighted curtains, and removable shower seat. Install two grab bars one 32 inches long and one 18 inches long, Install 48 inch wide vanity with 48 inches of counter space, 30 inches open leg space, one 12 inch drawer base, and single lever water control, Install 18 inch wide pantry cabinet, replace existing door and jamb with 36 inch door and jamb, install hardback board and ceramic tiles over existing subfloor, repair ceiling, move any walls to accommodate new fixtures and remove existing paneling on walls and replace with new insulation and green sheetrock. Ground-Fault Circuit Interrupter (GFCI outlet), Must be used. Shower/Tub area: Open concept at least 36 wide. Tile all walls, ceiling, and floor. Install or secure a bench in the shower. Install safety bars, shower heads, one stationary, and one hand-held. Install lighting and drain as needed. Install sink with base to allow wheelchair access. Install lighting, fan and electrical to sink area to meet the veteran s needs. Install tile flooring in bathroom to match shower tile.
