- Appendix A Solicitation #H98210-12-T-0012 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The Defense Human Resources Activity (DHRA) intends to award on a competitive basis. DHRA requires blackberries with voice and data service, air cards with data service and Wi-Fi hotspots with data service. DHRA utilizes blackberries to maintain communication via e-mail and voice with its staff. Air cards and hotspots provide DHRA staff internet connectivity for government furnished equipment (blackberry and laptop) while on travel. Internet connectivity is required to be able to access network resources such as e-mail and data. A detailed description of the requirement, the CLINs, proposal submission instructions, an all provisions, clauses, and addenda that apply to this acquisition can be found in the attached solicitation number H98210-12-T-0012. Solicitation number H98210-12-T-0012 is issued as a request for proposal (RFP). Please provide a complete proposal including: CAGE code, DUNS number, tax identification number (TIN), business size and Central Contractor Registration status. This requirement is being advertised on a competitive basis. The NAICS code is 517911 and the small business size standard is 1,500 employees. Contract award type shall be a Single-Award Firm-Fixed Price, Requirements type contract. The Period of Performance shall be a one year base of 18 June 2012 to 17 June 2013 with two 12 month option periods. Offerors shall submit their proposals in accordance with the submission instructions contained in the solicitation by 11:00 am EST (Eastern Standard Time), Wednesday, June 6, 2012. Only proposal received within the response timeframe will be considered by the Government. Offerors are responsible for downloading the solicitation, all of its supporting documents, and any applicable amendments posted on Federal Business Opportunities (FedBizOps) at http://www.fbo.gov. Amendments may be posted to FedBizOps up until the response due date. All requests for clarification/questions must be submitted in writing via e-mail to the Contract Specialist, Larry J. Helton, at larry.helton@osd.pentagon.mil. No information concerning this RFP or requests for clarification will be provided in response to telephone calls. Questions will be received up to 12:00 Noon Eastern Standard Time (EST) on June 4, 2012. Questions received after this time and date may not be answered.
