S. Coast Guard base Elizabeth City, North Carolina Building 97, (Facilities Engineering Building/Offices) 1664 Weeksville Road Elizabeth City, NC 27909 Wage Determination: NC 120056 1/6/2012 NC56 Disclosure of Magnitude: $0.00 - $25,000.00 Time of Completion: 60 Days after award of contract Site Visit: Only one organized site visit will be scheduled. Site visit is scheduled for June 20, 2012 at 1:00PM. 52.236-27 Site Visit (Construction) (Feb 1995) ALTERNATE (Feb 1995) (a) The clauses at 52.236-2, Differing Site Conditions, and 52-236-3, Sit Investigations and Conditions Affecting the Work, will be included in any contract awarded as a result of this solicitation. Accordingly, offerors or quoters are urged and expected to inspect the site where the work will be performed. Contractors are to contact CWO4 Henry Langford to schedule site visit. Evaluation Process For Proposals: The evaluation process will be comparison, only. Basis of Award: The government will award a firm fixed-price contract to the responsible offeror whose quote conforms to the solicitation, is fair and reasonable, and has been determined to the most advantageous to the Government, considering price. Award will be made on the basis of lowest evaluated price of quote. 1. SCOPE: Contractor shall supply all supervision, labor, equipment, parts and materials necessary to conduct a visual inspection of the sanitary sewer line in Building 97. Additionally, the contractor will conduct repairs as needed once approved by the Contracting Officer. This work is located at the USCG Base Elizabeth City, NC in building 97. 2. GOVERNMENT FACILITIES AND UTILITIES: Electrical and potable water are available on the site. The Contractor will be permitted to utilize these utilities in performing the work, provided that the existing systems are not overloaded. Telephone services will not be available for use by the contractor. 3. MATERIALS AND LOCATIONS: a) Materials shall be delivered to the jobsite in their original unopened packages, clearly marked with the manufacturer's name, brand name, and description of contents. Stored materials shall be protected from sunlight, and kept away from excessive heat. The contractor shall not store any highly flammable materials in the work area. Sufficient area for staging is available on site; storage areas will be pre-approved by the Contracting Officers Technical Representative (COTR). Special precautions shall be taken to ensure construction materials and debris does not enter the airfield, runways, or taxiways. b) The Contractor is required to comply with local VOC laws and regulations. All applicable Material Safety Data Sheets shall be available upon request. 4. SCHEDULE OF WORK: Completion of work shall be accomplished within 60 calendar days from acceptance of purchase order. The Contractor shall perform all work during normal base work hours. The normal base working hours are 8:00 am to 4:30 pm, Monday through Friday, but the contractor is not limited to these hours. If the contractor is going to work outside normal working hours, permission must be obtained from the COTR. 5. EXISTING CONDITIONS: a) The contractor is expected to satisfy himself as to the existing conditions and the work to be performed. The contractor shall verify all dimensions and equipment data prior to ordering materials and commencement of work. b) The existing sanitary sewer line from the furthest clean out port to the manhole is believed to be the original line installed in 1943. It is a 4 to 6 inch diameter cast iron line approximately 250 feet in length. 6. LIABILITY OF CONTRACTOR: Any damage to government property caused by the contractor shall be repaired or replaced by the contractor at no cost to the government. The contractor shall not disconnect any service without approval from the COTR. 7. INSPECTION AND ACCEPTANCE: The COTR will accomplish inspection and acceptance of work. 8. LABOR LAWS: The Davis-Bacon Act applies to the procurement. 9. SAFETY: The Contractor is wholly responsible for work site safety. The Contractor shall implement a safety program that protects the lives and health of personnel in the construction area, prevents damage to property, and avoids work interruptions. To accomplish this end, the Contractor shall provide appropriate safety barricades, signs, signal lights, etc. as well as complying with the requirements of all Federal, State and Local safety laws, rules and regulations. 10. SITE RESTORATION & CLEAN UP: The work site shall be cleaned up at the completion of each workday. Contractor shall take special care to contain dust while working. Upon completion of the job, the Contractor shall clean up the job site, returning it to a state of cleanliness equal to or exceeding that in which it was found. The Contractor shall properly dispose of any trash, extra materials, dirt, debris, or other litter that remains. There will be no final acceptance of the contract until the Contracting Officer's Technical Representative approves the appearance of the job site. 11. GOVERNMENT FACILITIES AND REGULATIONS: The Contractor, their employees, and any sub-contractors shall become familiar with and obey all Base/ Unit regulations. All personnel employed on the project shall keep within the limits of the work area, and shall not enter any other area outside of the work site unless to do so in the performance of their duties. There shall be No Smoking in any Coast Guard Building. 12. CONTRACTING OFFICER'S TECHNICAL REPRESENTATIVE: The Contracting Officer's Technical Representative (COTR) will be designated at the notice to proceed. 13. TECHNICAL REQUIREMENTS: Task #1: Provide inspection services for approximately 250 ft of sanitary sewer. a) The sanitary line in question runs from the manhole located to the west of building 97 to the break room clean out port, noted on the attached drawings as manhole and break room. The intent of this inspection is to find out the condition of the sanitary sewer in question and identify possible causes of the deteriorated flow through the line. b) Remove all foreign objects from the sanitary piping with a cable/chain machine. Hydojett sanitary piping to remove accumulated debris. c) Line inspection to include running a video camera through the entire sanitary piping system (from manhole to the end of line) in question to determine the condition of the piping. The video camera to include a built in transmitter and walk above ground locator which shall be used to sketch all sanitary piping. d) Sketch the sanitary piping on attached drawing including dimensions. Show the sewer piping from the manhole to end of line (Break Room) as noted on the attached drawing. e) Locate all branch lines off of the main line and provide description (diameter, type material, etc.). f) Provide brief written description of findings to USCG Base Elizabeth City, NC Facility Engineering Department. Include a video DVD and drawings with a sketch of the location of all sanitary piping on the attached drawings and location of any damaged piping. Reports shall be delivered to Mr. David Aydlett. Task # 2: Provide all supervision, labor, equipment, parts and materials necessary to repair a section of sanitary sewer line ten (10) feet in length. a) Repair will include all necessary construction needed to conduct repair including, but not limited to: location of the deteriorated area, saw cutting & demolition of existing cement slab, excavating surrounding area for access, repair/replacement of existing pipe with compatible materials, proper back fill and compaction, and replacement of concrete flooring to existing conditions. b) All equipment that may be above the section needing repair will be disconnected and relocated by the government. c) The quantity of this task order will be determined by the initial inspection (Task 1) and awarded as needed. d) Task # 3: Provide all supervision, labor, equipment, parts and materials necessary to clean and re-line approximately 250 foot section of sanitary sewer piping. a) Re-line the existing sewer line and all repaired sections with a non-invasive liner which has a minimum life expectancy greater than 25 years. 14. TEST AND PERFORMANCE: The Contractor shall notify the Contracting Officer's Technical Representative (COTR) 5 days before the performance, inspection and acceptance tests are to be conducted. The tests shall be performed in the presence of the COTR. 15. EXECUTION: Installation, workmanship, fabrication, assembly, erection, examination, inspection and testing shall be in accordance with appropriate sections of 29 CFR 1910, 1926.
