Attachment 2 Attachment 3 Attachment 1 Solicitation This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items, and FAR Part 15-Contracting by Negotiation, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Proposals (RFP) NIHLM2012356. This solicitation document incorporates provisions and clauses that are in effect in the FAC 2005-58 FAR Revision effective May 18, 2012, including all FAR Circulars issued as of the date of this synopsis. This acquisition is a Total Small Business Set-Aside and the North American Industry Classification System (NAICS) code is 541690, Other Scientific and Technical Consulting Services. It is anticipated that ONE AWARD will be made from this solicitation. This requirement is for a base year with options for two (2) successive one year periods. The period of performance will begin on or about August 15, 2012. **The Solicitation, Statement of Work (Attachment 1), Electronic and Information Technology Accessibility (Attachment 2), and Buy Accessible Government Product/Service Accessibility Template (GPAT) are attached to this solicitation.** All proposals received by July 11, 2012 at 12:00 P.M. (Eastern Standard Time) will be considered by the National Library of Medicine (NLM). Responses must be in writing, and must include pricing information and meet the criteria requirements. All responses from responsible sources will be considered. Inquiries regarding this procurement may be made to the National Library of Medicine, Office of Acquisitions, 6707 Democracy Boulevard, Suite 105, Bethesda, Maryland 20892-5488, Attention: Shari Shor, Contracting Officer, on (301) 435-4388. Please submit ONE (1) signed original and FIVE (5) copies of the proposals on or before 12:00 P.M., EST, July 11, 2012 to: Shari Shor Contracting Officer National Library of Medicine, NIH Office of Acquisitions 6707 Democracy Blvd., Suite #105 MSC 5488 Bethesda, MD 20892-5488 All proposals received within thirty (30) days from the date of publication of this solicitation will be considered by NLM. EMAILS AND FACSIMILES WILL NOT BE ACCEPTABLE. Firms interested in responding to this notice must be able to provide the products and services described by NLM in the Statement of Work. Vendors shall comply with all applicable Federal, State and Local laws, executive orders, rules and regulations applicable to its performance under this order. The following clauses and provisions cited herein are incorporated by reference into this solicitation and maybe obtained from the website http://rcb.cancer.gov/rcb-internet/SAP/sap.htm: FAR 52.212-1, Instructions to Offerors-Commercial Items (June 2008), FAR 52.212-2, Evaluation-Commercial Items (January 1999). Offerors are advised to provide with their offer a completed copy of the following provisions: FAR 52.212-3, Offeror Representations and Certifications- Commercial Items (May 2011)-With DUNS Number Addendum [52.204-6 (April 2008)]. The following FAR clauses apply to this acquisition: FAR 52.214-4, Contract Terms and Conditions-Commercial Items (June 2010)-With Addenda [Stop Work Order, FAR 52.242-15 (August 1989) and Year 2000 Compliance (July 1997)]; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (August 2011), as well as the following clauses cited therein: FAR 52.232-18, Availability of Funds, FAR 52.219-6, Notice of Total Small Business Set-Aside (June 2003), and FAR 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration (October 2003).
