This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ) No, F1D0152101A002. The solicitation document; incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-59. This requirement is set aside 100% for small business. Performance: 1 September 2016- 30 August 2017 Total Value base with four options:___________ STATEMENT OF WORK (SOW) Protestant Religious Education Coordinator JOINT BASE ANDREWS NAF WASHINGTON 1. SCOPE OF WORK: The contractor shall furnish all labor, tools, materials, transportation, and incidentals, except as specifically provided, as necessary to perform the duties of the base Chapel Protestant Religious Education Coordinator position in accordance with the tasks identified herein. Contractor must exhibit a positive, self-directed and motivated disposition. In addition the contractor will coordinate, administer and plan all religious education programs and monitor bible studies sanctioned by the chapel through the Senior Protestant Chaplain and Deputy Wing Chaplain for clarification and information. The Wing Chaplain has final approval of all religious materials. 2. SKILLS: The Protestant Religious Education Coordinator must have the following: 2.1. Interpersonal. Contractor must constructively work with chapel staff, parish staff, parishioners, and the public in an interfaith arena. 2.2. Professional. Contractor shall be an efficient manager and able to coordinate activities, and facilitate communication between parish organizations. 2.3. Communication. Contractor shall be able to communicate the needs and goals of the Protestant Religious Education Program to chapel staff, volunteers, parishioners, and the public, both verbally and in writing. 3. SCOPE OF RESPONSIBILITIES: The Protestant Religious Education Coordinator must: 3.1. Develop annually a continuing religious education program for the Protestant parish to include but not limited to: Sunday school, Children's Church, Approved Workmen Are Not Ashamed (AWANA), chapel sponsored bible studies/classes, discussion groups, Vacation Bible School (VBS) and other meetings or activities dealing with religious education. 3.2. Develop an annual budget (1 Oct - 30 Sep) in writing for all religious education requirements; to include: selecting needed materials, publications, supplies, projected programs, teacher training and special religious education programs. 3.3. Develop and plan for registration activities, recruitment and training of teachers (including substitutes and helpers) and ensure all adult workers (18 years or older) have the prescribed installation records checks completed, kept current and on file. 3.4. Attend monthly Protestant Parish Advisory Council meetings (approximately one hour), providing religious education reports as directed by the Senior Protestant Chaplain. 3.5. Prepare religious education publicity as required for base paper, official base bulletin, and chapel bulletins at least three (3) weeks in advance of the activity or program. Publicity is to be submitted to the Wing Chaplain for final approval in final written format for printing. 3.6. Submit all religious education fund/purchase requests to the chaplain who is supervising the religious education program. The requests should be submitted at least six (6) days in advance for purchasing religious education items. 3.7. Attend weekly chapel staff meetings (approximately one hour) and report weekly updates. 3.8. Coordinate to ensure the required facilities are open and ready for use thirty (30) minutes before starting time. 3.9. Coordinate and provide oversight for all evening and Sunday morning classes. Ensure facility requests are submitted in a timely manner for any events in the chapel. 3.10. Recruit and train teachers and volunteers to implement Vacation Bible School (VBS) as requested by the Protestant Chaplain. 3.11. Work with the Catholic RE Coordinator for ecumenical VBS, as needed. 3.12. Establish a program so teachers contact all absentees. Inform the Protestant Chaplain about students with special needs, which might require his/her personal contact/visit. 3.13. Maintain current information for all religious education program participants including name, address, phone number, age (exclude adults), marital status/single parent, and any special needs for each group (must be maintained in accordance with Privacy Act requirements). 3.14. Monitor the handling of all funds received in the RE program to ensure compliance with applicable directives (Coordinate all funds i.e. bank bags, etc. with Chapel Tithe Offering Fund (CTOF) Account Manager). 3.15. Ensure all RE workers are aware of local safety requirements and procedures. 3.16. Secure and clean all facilities utilized in conjunction with religious education programs. 3.17. In accordance with the requirements of the DOD Instruction 1402.5, Criminal History Background Checks of Individuals in Child Care Services, the Service Provider must submit all requirements (fingerprints, biographical data, background information, etc.) necessary for completion of national, state, and local law enforcement agency background checks. Questionable data obtained as a result of the background check will terminate contract. 3.18. Compile and maintain a continuity file on each RE program/event (ensure it is on file plan). 3.19. Keep records of students and teachers on appropriate Air Force forms and provide statistical input for the AF 1270, with all Religious Education data. 3.20. Responsible for finding a substitute who can fulfill this contract in both scheduled and unexpected absences. 4. EDUCATION AND EXPERIENCE REQUIREMENTS: Protestant Religious Education Coordinator must: 4.1. Have completed an academic degree or have a background in education 4.2. Have education experience within a chapel setting. 4.3. Be active or willing to be active in the Protestant Chapel Program. 4.4. Become visibly known within the Protestant parish by attending worship services and programs 4.5. Submit a resume to include certificate(s) and work related references. 4.6. Demonstrate enthusiasm for religious education, parish ministry, and work with chapel staff, contractors, volunteers, and laity. 5. TIME AND PLACE OF PERFORMANCE: 5.1. Provide services of 25 hours per week September through May (each hour equals 1 unit). 5.2. A requirement of 15 office hours at a minimum Monday through Friday 0730-1630. The Wing Chaplain may reduce office hours to meet the religious education needs after normal duty hours. 5.3. Coordinate all support requirements with the CTOF Account Manager and/or the Appropriated Funds Manager. 5.4. Contractor will be provided a work space Chapel 1. Government-owned and chaplain tithes and offering fund (CTOF)-owned property such as overhead projectors, TV/VCR, tape recorders, etc. are available for RE use. Government-owned equipment will not be moved without the permission of the Wing Chaplain and/or NCOIC. CTOF-owned equipment will not be moved without the permission of the Account Manager. Requirements for materials/supplies will be submitted on Purchase Order Request forms to the Account Manager, and will include: item/title, quantity, cost including shipping/handling), and vendor (including address, fax, and phone numbers [800 number if possible]). Period of performance shall be a base period of 12 months with four 12-month option periods. Interested parties must submit clear and convincing documentation the capabilities to satisfy the requirement specified above and in Performance Work Statement (PWS). Only written responses will be considered. Required delivery location is at the 11th Wing/HC, Chapel One. The Government intends to make a single purchase award to the responsible contractor whose quote will be the most advantageous to the Government. The Government will select the best overall offer based on Lowest Price Technically Acceptable (LPTA). Only those offerors determined to be in compliance with the requirement as stated in this solicitation will be considered for award. When evaluated, all evaluation criteria will be weighed equally. If additional information is required to support price fair and reasonableness, each offeror may be required to provide a hard copy of their price breakdown. Non-conformance with this requirement may result in the offerors quote being determined unacceptable. The government also reserves the right to open each competitive offer for discussion if it is determined to be in the best interest of the government to do so. No partial quotes.
