- The following modifications have been made to this notice: Phase I Technical / Company Requirements During the pre-source selection procedures, CPT approved the company/technical requirements outlined below. The requirements below must be answered with the vendor's initial submittal and required documents by the response date stated in the FBO announcement. A vendor who does not meet these requirements or who does not submit documentation to support these requirements with initial submittal will be disqualified in Phase I. 1. Vendor must provide a complete line of Syringe products. a. At a minimum the vendor must provide sterile, single use, disposable plastic, and latex free/safe, syringes with bold and clear scale markings of the following sizes and tips: 1 mL (cc) Luer Lock Tip, 1 mL (cc) Slip Tip, 3 mL (cc) Luer Lock Tip; 5 or 6 mL (cc) Luer Lock Tip; 5 or 6 mL (cc) Non-Sterile Amber Oral Syringe with Tip Cap; 5 or 6 mL (cc) Non-Sterile Clear Oral Syringe with Tip Cap; 10 mL (cc) Non-Sterile Amber Oral Syringe with Tip Cap; 10 mL (cc) Non-Sterile Clear Oral Syringe with Tip Cap; 10 or 12 mL (cc) Luer Lock Tip; 20 mL (cc) Luer Lock Tip, 30 or 35 mL (cc) Luer Lock Tip; 60 mL (cc) Catheter Tip, 60 mL (cc) Luer Lock Tip. 1 mL (cc) Luer Lock Tip Syringe 1 mL (cc) Slip Tip Syringe 3 mL (cc) Luer Lock Tip Syringe 5 or 6 mL (cc) Luer Lock Tip Syringe 5 or 6 mL (cc) Non-Sterile Amber Oral Syringe with Tip Cap 5 or 6 mL (cc) Non-Sterile Clear Oral Syringe with Tip Cap 10 mL (cc) Non-Sterile Amber Oral Syringe with Tip Cap 10 mL (cc) Non-Sterile Clear Oral Syringe with Tip Cap 10 or 12 mL (cc) Luer Lock Tip Syringe 20 mL (cc) Luer Lock Tip Syringe 30 or 35 mL (cc) Luer Lock Tip Syringe 60 mL (cc) Catheter Tip Syringe 60 mL (cc) Luer Lock Tip Syringe Phase II Acceptability Evaluation Clinical Performance Vendor must be able to deliver product literature, training materials and evaluation samples as requested to the MMESO Pacific via FedEx or UPS AND to the MMESO Europe via United States Postal Service (USPS) Priority Mail with Certified Mail Service and throughout the United States by selected shipping method when requested in Phase II. Vendor must send the MMESO North verification regarding the shipment of literature and samples. This information will include the date of shipment, final delivery date, tracking numbers and the carrier used by the vendor. Vendor will provide only the following no cost product samples for clinical/performance evaluation. Unused samples will not be returned. One (1) box (at least 25 each) of Syringes of each size with various style tips (Slip, Luer Lock, and catheter) listed below are to be provided to each of the evaluation sites as identified in the letters of invitation. Items to send 1 mL (cc) Slip Tip Syringe 5 or 6 mL (cc) Non-Sterile Amber Oral Syringe with Tip Cap 10 or 12 mL (cc) Luer Lock Tip Syringe 60 mL (cc) Catheter Tip Syringe Clinical/Performance Requirements: During the acquisition planning process, the MMESO CPT approved the clinical/performance requirements outlined below. 1. (Patient Safety NPSG Goal 3.) The gradient scale system on syringes should be clearly marked/ readable to prevent medication administration errors. The gradient scale system must be readable/clearly marked to assist in making accurate dosage measurements 2. (Patient Safety /NPSG Goal 3.) The gradient scales on the syringe should accurately match the volume of solution in the syringe barrel. The gradient scales on the syringe must accurately reflect the amount of solution in the syringe barrel so accurate medication dosages can be delivered and prevent medication delivery error. 3. (Patient Safety; NPSG goals 3) The syringe should have smooth and precise movement under a constant amount of pressure/force. The syringe plunger must move smoothly under a constant amount of pressure/force to allow the syringe contents to be dispensed without injury to patient.
